Playing when I'm sick usually makes me feel tired. So, if I do game when I'm sick, I don't do it for more than around 20 minutes before I give up and take a nap.

I don't see how these things promote game sales. If you want a game, you'll get it. If you don't, you won't. They really only promote pre-orders. No one is going to see a L4D2 commercial promoting a special baseball bat deal and say "My negative opinion on that game is now completely swayed...

I guess it depends on what type of Karma you're going for, but either way I wouldn't do it. Megaton makes up alot of gameplay. Plus, if you're a bad dude Jericho will join you on your adventures. I found it more fun to slaughter everyone in Tenpenny Tower.
