The game was Rolemaster.
One of the old school, simulationist systems that has a reputation (entirely deserved) for complexity and oh so many charts and tables.
Combat is slow but detailed and quite, quite fatal.

I too get misery boners and frustrated at a happy ending on occasion, but I also grew up in England during the eighties.
