Fallout 3 Mod Location Help

Okay, I've seen this custom HUD for Fallout 3 floating around in images and I can't find it anywhere. I'd like to know where to get it.

I like the time/date information. What are the % for? I'm going to look around a bit.

I don't think it would be hard to create a custom mod like this, if you wanted to tweak some of the information displayed. I might look into doing that too. I never thought of putting the effort into doing my own UI to display what I want to see.
