This reaffirms my belief that she is turning pop music back into an art form, rather than just a money-making-machine. The whole middle of it was like pornography, so she'll likely release a clean version for TV, but damn! she is pushing boundaries. This girl is turning pop music on it's head, and all I can say is "FINALLY!"
I did enjoy the religious and military imagery as well. I heard she was raised Catholic, so juxtaposing her in habits with the giant orgy could show a growing point for her. I can't think of any immediate meaning for the military parts, but I know it was amazingly choreographed.
Also, I hope I don't hear much about the length of it. Telephone's full video was over 10 minutes (I never made it through, but the 4 minute one was fun to watch), so this is unquestionably shorter. At least this one had plenty of content and kept the music continuous.
All in all, I think it's a great success for her. She's gone a long way from Just Dance and her grossly overestimating her sex appeal. If only she didn't get so overplayed, I might listen to her more.
