I sent out Jack as the biotic shield during the swarm bit while not loyal. Only team member I couldn't care less about. I felt nothing!

The first time I played, Tali died. That sucked, since she was my favorite character.

Every subsequent playthrough, everyone has gotten out alive. Everyone. Every time.

Those playthroughs total nine. (So far.)

Yeah, I'm such a fanboy.

I lost Mordin simply because he can die really, REALLY easily if you leave him behind when going to fight the main boss. I simply took him along next time and everything was cool.

The choices weren't that hard to make, really. You know that Garrus commanded a good squad of his own, so he'd be a good team leader. You'd also know that Grunt would be a terrible team leader. For hacking the door, I can't understand why ANYONE would pick anyone other than Tali or Legion. One is a tech person, the other is basically a computer with legs. As for the biotics thingamajig, Jack or Samara is the logical choice, because the game explicitly states that they are really powerful biotics.

But the game doesn't say that Mordin has a high chance of dying if you don't take him with you. He doesn't always die, but he's got a chance of dying, which sucks. I immediately reset the game when he died, having saved just before the final choice. I pretty much refused to have everyone die.

Saved everyone the first time through, obviously.

My personal 'canon' playthrough kills off Jack, Thane, Jacob, Tali, and Kasumi.

In all my playthroughs, I haven't lost a single teammate. I always make sure to upgrade my ship and do everyone's loyalty mission.

On my second playthrough I let Jack do the biotic shield since I wanted Samara available for reave support. Then the seekers flew off with Wrex - WHAT?!?

-do the loyalty missions
-upgrade the ship
-put squad members with leadership experience in charge of B teams
-balance your teams, one specialist each (tech & biotic)
-if you lose Jack's loyalty, don't put her in charge of anything

Easy, maybe too easy? Once you know the formula, it kinda sucks the tension out of subsequent playthroughs.

I lost 2 when I did it without guides. Then of course I reloaded right before the mission and did it with a guide.

Zeed died because I assumed an old dude would do alright leading a squad (apparently he's not up to it). The secretary chick died because I did some side quests before the final mission.

I didn't lose anyone. You probably made poor decisions during the mission or didn't make your squadmembers loyal to you.

First time I only lost one person, unfortunately, that person was Mordin. I didn't really think it through I guess, since I put him as the tech specialist.
Hey he's a scientist right? He'll be fiiine.

I didn't have any squad members die in the final assault. I just made the most logical choices when asked who to send where. Pro-tip: Send Mordin back to escort the rescued crew to the Normandy if you don't plan on having him in your party.

I lost Miranda... She wouldn't shut the hell up about being in command or some fire team specialist and kept making herself sound so self righteous and important the whole mission. So at the end, fuck fine. You want the job so bad? Take it.
There we are all running to the ship with the weapon and survivors, and there's Miranda face down in the mud. Can't say I will miss her if she were to be in the third one. She just pissed me off the whole game with her "I'm the greatest thing to ever walk this land, worship me!" attitude."

I have absolutely no idea what I did wrong, but I ended up losing Jack. Didn't care though, that ***** had it coming.

There should have been a separate achievement for committing suicide by intentionally botching (or should that be succeeding completely) the 'Suicide Mission' (maybe called "Tried Suicide" or "Final Mission Accomplished").
Had it not been for Youtube, I wouldn't know what it is like to lose a party member.

Lost Mordin. Didn't want to risk Tali in the vents. Figured Mordin's scientist background might give him an advantage in the technology area

I had ever Squad member loyal, I lost Thane, Jack, Jacob and Zaeed.

Lost Thane to the Seeker Swarms... It was a sad day.
