Another vote for "Audition."

Also, "Begotten";

Not the worst, perhaps, but hard to sit through.

Kiri kiri kiri kiri.. !

Until I decide the most disturbing movie I've ever seen, I'll leave you with a story of the most disturbing moment. I used to work until one a.m. sunday night, sleep for about 4 hours and turn around and work the morning shift on monday. I would take a couple sleep aids to help fall asleep. One night I couldn't sleep so in a sleep aid-induced haze i walked into my living room and turned on the t.v. Now my t.v. was a little older so I heard what was on before the picture came on. I heard a clinking noise and wondered what the hell it was. The first image my sleep aid and underslept eyes saw was the "curbstomp" scene from American History X. I knew then that sleep was going to be impossible that night and the image has stuck with me to this day.

Irriversible - It has a 15 minute long rape scene.
Tideland was depressing crap, I wouldn't necessarily call it dusturbing though.
Them was a pretty good horror movie, the monster is definitely disturbing

2 Girls, 1 cup. Also a friend told me about a new video called "2 kids, 1 sandbox." If it is what he says it is then you should not search for it.
Never, EVER watch "Kids in the sandbox".
It's the first thing I've ever watched that has caused me empathetic bodily pain.

The japanese film Suicide Club, where at the beginning of the film some happy circus music plays in the background while 50 young schoolgirls holding hands count to 3 then jump in front of a speeding metro train. And it doesn't get any better from there on.

An American Crime, starring the lovely Ellen Page.. only it's completly fucked up, like most of Ellen Page's movies.

Urotsukidoji. Avoid it at all costs. Got to be the weirdest anime I have ever seen. It was very disturbing. The plot didn't make any sense either.

When I was 8 or so the most disturbing film I saw was Evil Dead 2 - I had nightmares for about a week.

For the record I have watched and enjoyed it since.

I dunno for artistic value I thought it wasn't too bad. The plot was weird to say the least but the idea was kinda cool, at least I thought so anyway.

Though possibly a little too much gratuitous sex, gore and violence.

Once upon a time, I was flipping channels at one in the morning. Suddenly, I came upon something interesting. It was a porno on HBO involving candlewax, plastic rap, leather, possible transvestites, and bad acting. I was scarred for life.
