Neither of them are the goriest thing I've ever seen, far from it, but the way in which it is carried out will deeply disturb you.
Oh yeah especially the ending of School Day's that was so fucked up and creepy but so awesome at the same time.

Just thought of 2 others, but they're actually documentaries, still they réally freaked me out:
Jesus Camp, a docu about the extreme fundies in America, seeing those kids REALLY freaked me out.
The other one would be a docu about North Korea, some eye-surgeon who was allowed in the country. The whole thing was simply...unreal, especially one scene after people could remove there bandages (after surgery) for the first time, you just can't make up shit like that.

Tbh, I think those are scarier then most movies I've seen, for a simple reason: that shit is réal.

Yes, thank you. From the get-go when I saw this movie in threaters, I had to pee. I was sitting there thinking, "damn, the movie just started, and I have to pee." I didn't want to get up, else I'd miss something. So, through the whole movie, I sat there with my bladder clenched so hard, that ever fright and scare made my body clench in twisted ways! So not only was I scared from the movie, I was scared I'd miss something cool (I would have) and/or wet myself!

Happy ending-- the moment those credits began to roll, I pulled mach 6 to the restrooms. :)

Disturbia was disturbing, cause of the fact that it wasn't scary, same thing with The Happening.

I let off alot of WTF's, when watching Zack and Miri Make A Porno.........

I hit this thread thinking, "Hey, I can get some idea of a crazy movie to watch." Now, after reading some of the twisted shit people have seen, I feel kinda queesy, and don't really want to watch anything messed up anymore.

I will never watch another Shaymalan movie ever again. The Happening was so bad! I'm ashamed to admit that I watched it because of the advertised, "first R rated movie." That should have been a warning light for me. He never gives enough information in his previews, and I always walk away shaking my head. And here? Rock bottom-- the plants are killing people, so lets all run away? I guess B movies really arn't dead.

Teeth and Pan's Labyrinth. Holy shit, wtf? A chick with a chip on her shoulder and teeth in her vagina? And Pan's Labyrinth? I thought I was watching a childrens movie until that guy beat the face of the rabbit hunter in, then that fucking fairy eating thing?! Holy SHIT!

I've never really been disturbed by a movie, I laughed my way through Cannibal Holocaust apart from the animal death scenes, did they have to spend QUITE so long disembowling that turtle? I imagine I would find Salo: 120 Days Of Sodom very disturbing but luckily I have no intention of seeing it.

Oh yeah I just remembered, Epic/scary/date/disaster movie, it disturbs me that people are stupid enough to laugh at these horrible excuses for films.

There's no full movie I've seen that's really freaked me out, but I was watching this crappy horror film called 'They Nest', basically about these weird cockroaches who use humans to incubate their eggs.

There's this one bit where a guy gets swarmed by these bugs, paralysed by them, then while still conscious has this really huge gross slug-type thing crawl down his throat into his stomach. Just thinking about that happening to you is just horrible.

Also, on the point of general horror, I find the stuff from the 70's and 80's a hell of a lot scarier than most modern films. I think it's because the gore from then is kind of midway realistic, more than comical but less then real, which somehow makes it seem a hell of a lot worse.

Darren Aronofski is just plain weird like that, have you seen Pi? That movie is seriously fucked up, but not in the same "obvious" way that Requiem was...it was just weird seeing Jennifer Connoley doing what she was doing...I had such a crush on her going all the way back to Labyrinth and re-enforced by Dark City...oh well...The Wrestler was actually pretty "normal", though, I guess.

Oldboy, Pi, Kind-Hearted Ms. Geum-Ja (lady vengeance)

Oldboy is simply brilliant and sick in most ways possible :D and the hammer - corridor scene is one of the best fighting scenes in the history of cinema.

Pi was fine as an brain fuck but lacked depth to it. in the end you was left with no explanation what so ever. good movie however great soundtrack and atmosphere

In kind-hearted ms. I loved the prison scenes :D there was something disturbing in each one
and of course the execution scene creep me out.

Edit:

Forgot to mention Imprint from the masters of horror series by Takashi Mikke. Not much sense and story but disturbing as hell and leaves a mark on your brain to the rest of your life.

Pink Flamencos by John Waters. That movie is just surreal (like most early John Waters flicks) but this one takes the cake because of THAT SCENE (those who've seen it know exactly what I'm talking about).
