Bleed and Frost proc is OP in ER, as well as some Fire or Flame attacks especially when used for resets in conjunction with the former. Jump attacks can be too depending on gear. However this is compensated in some cases by bosses being total dbags with either health regeneration, huge combos or one shot attacks, poise for days, or sometimes all of them combined!

A lot of the electrical abilities once you get them in Evil West. The default parry already has a generous window too.

Fully upgrading the Riot Gun and Sword in Shadow Warrior 3. The Riot Gun at max upgrade allows you to keep firing without reloading. The Sword allows the player to charge it and release an elemental attack that will freeze or stun-lock demons. It's randomized between fires, ice, and lightning, but you want the latter two elements.

The Zenith in Terraria. Mind you, you can only naturally get the damn thing after beating the final boss in the game and even the requires a bit of luck/grinding to get the necessary components, so at that point, there's nothing to really challenge you anyways. But good lord that thing shreds. Strongest weapon in the game (theoretically capable of destroying the final boss in 11 seconds), ignores invincibility frames, ignores blocks and extends to your mouse location (mind you, it's somehow still considered a melee weapon), homes in on nearby enemies, and even acts as a brief light source that helps you fill in the empty parts of your map.

The "Refreshment" perk in The Witcher 3.

It does require a bit of leveling up before you can unlock it so you can't get it right away but especially with the latest upgrade, you can actually equip it by the time you're done with the Bloody Baron (you need to invest 6 skill points into other alchemy skills first).

This perk gives you more vitality (health) when you drink any potion. Since you can drink potions during combat, and replenishing potions is super easy with the game's much simplified alchemy system compared to previous games, and you can quick slot potions, this basically makes you invincible as long as you're facing enemies at your level.
For example, quick slot the Thunderbolt and Tawny Owl potions for strength and stamina acceleration- the first two potions the game gives you for free- and pop them while fighting and you're stornger, can cast spells faster, and at full health all the time. Now you broke the game.

Many of the alternative Special and Blitz Attacks unlocked in Streets of Rage 4: Mr. X's Nightmare. Blaze's and Floyd's being the most game breaking. Shiva meanwhile is a gamebreaker no matter what you have equipped, but his second Blitz attack makes him nearly invincible.

Update with the "next gen"... er.. update..
They made getting to those higher level perks easier. Each level within a tree requires having had already invested a certain number of total points in that tree to unlock them, and now they made it require less. They also lowered the amount of possible upgrades for some, to balance.

So before the next gen update, you would need I think 8 points to unlock Refreshment IIRC, so I would put 5 in Poisoned Blades and 3 in Acquired Tolerance. That means I wouldn't get to Refreshment until well into ore maybe after the Bloody Baron questline, which is the hardest part of the game anyway.

On the next gen play-through. you only need 6 points to unlock Refreshment, and none of the skills allow more than 3 upgrades anymore (but the 3rd is as strong as what was previously the 5th so nothing is sacrificed). These changes allowed me to unlock Refreshment before even starting Velen.
An additional bonus is that you get green mutagens first, which gives you an additional vitality bonus if you equip green skills.
So- you get to Velen, you have a green mutagen, Poistoned Blades, Refreshment, and Frenzy (to slow down enemies when you drink potion), and you're already practically invincible.

Whatever, just wash your hands.
Serious Sam 4/Siberian Mayhem have a few op weapons, or at least they would be op in any other game, in Serious Sam they just hold back the horde for longer then other weapons. In SS4 we have the death ray attachment for the laser gun, it burns through ammo but it kills almost anything really quick, if you have a flat area you can just kill a huge number of kleer with a sweep of the death ray and it burns down the big guys too. The crossbow in SM also falls into this category, its a sniper weapon, it hits harder then the rocket launcher, it goes through enemies a single headshot will take down anything lesser then a khuun, it makes railguns in other games look weak.

Not a 'real game' but a mod. But basically Halo 1 has this mod called Cursed Halo, and it recently got updated to something easy to install through Steam Workshop with MCC and with a lot more shenanigans.

It's a lot of jank and silliness but by god do I love the Giant Magnum when you do find it. It's absolutely overpowered but it's such a joy to use.
