I'm not sure about the first two steps but I didn't need to get the new achievement I just started a new game on the last chapter and got this new ending.Ricotez said:You need several things before you can see the alternate ending:

You'll then hear a weird sound instead of the usual Still Alive elevator music loop. As far as I know there's no way to revert Portal back to the original gameplay, however the fact that you need 'Transmission Received' to see the new ending, you need 'Heartbreaker' to earn 'Transmission Received', and you need to have completed the game at least once for 'Heartbreaker' means that new players will get to see the original game before they play the alternate version with the radios and the new ending. A very wise decision of Valve.[/spoiler]
