My most-played game is Super Mario Land for the Game Boy. It has been played at least 5 times a year when I'm bored.

OT: Warcraft 3. I have played DotA for a good 3-4 years I think. So, yeah, it's cool but lol and stuff. I have, however, moved on to HoN now, so, make a new thread in 3-4 years and I will post HoN instead.

I could get technical and say DOA, since each 20 minute character story arc could count as a play through...but I think I will have to go with Half Life, original. Probably played it through a good 7 times or so. Good enough to play pretty often, short enough that its not a huge expenditure.

My game with the most restarts is between KOTOR and Oblivion. Both at least 30+ times. Except i cant check because i accidentally deleted everything on my hard drive a while back
