One game from 1st, 2nd, and 3rd generation of Pokemon.

Also LEGO Lord of the Rings... not sure why, but I did it.

None. I came close to 100% on Okami but that Kai race kept kicking my but to the point that I finished the game without.

None.

I've probably got only 1 achievement remaining on 3 or 4 games, but it's always something that makes me think "I'm crazy, but not THAT crazy.".

Just Cause 2.

Bahaha had you going for a moment there didn't I?

Most of the must haves on N64 I've fully unlocked. Not Super Mario 64 though, because I got to 108 stars and lent it to somebody and the bastard finished the rest off... and I forget which ones I had left to do.

When I was younger I 100%'d Spyro: Year of the Dragon several times. I also did Pokemon Red once. I did a 100% run of Zelda: OoT while only taking 6 hits. I also did a 100% run of Kingdom Hearts 2 back in 2008. For achievements, I think Bioshock and Fallout 3 were the only ones I got 100% on.

I got 100% on the first Drake's Fortune game. That was before they stupidly decided to enforce multi-player on you.

Red dead redemption.
Seriously, I would marry that game. I've finished the normal mode 5 times, 100% 3 times, Done undead nightmare 3 times, and finished that 100% twice.

10 completed with 100% gamerscore:

Mass Effect 3 (Nothing really too difficult)
Minecraft (Hardest one was the minecart achievement and even that was fairly easy)
Walking Dead (Just did the story)
From Dust (Fun game, the challenge mode was indeed challenging but I am currently in the top 100 on the leaderboard)
Portal: Still Alive (Keeping track of portal placement was necessary for Out of the Blue)
Mass Effect 2 (Nothing too challenging once you get the game down)
Oblivion (Really disappointing achievement list to be honest)
Harm's Way (Basically 200 free points, I recommend it just for that)
Assassin's Creed 2 (Those fucking feathers)
Fallout 3 (Fucking Mothership Zeta and its motherfucking audiotapes)

I also got 100% sync on AC:Brotherhood, but didn't play much multiplayer. That fucking tank section took me 20 tries and each one took at least 20 minutes and was instant fail. Seriously, fuck that tank.

Achievements wise (XBL/PSN)? Oblivion and assassins creed 2. I'm not really much of a hunter.

Actual completions? where there's nothing left to do?(old platforms and PC) Too many to count.

Spyro 2 & 3
Terraria (every item obtained)
Mass effect 2
THPS2 (those ps1 days of owning like 3 games total)
Fire Emblem GBA (Oh god I want awakening to come out now. NOW)
Halo 3 (every obtainable mutliplayer armour, but not all the achieves at once, some were on a 2nd account, everything done at least once though)
... I don't even know how many, these were the first I thought of for some reason

NEARLY done; so fucking many. Most infuriatingly TWEWY but I'm just not willing to do the pin level grind and farm the rarest items. Just no.

Fallout 3
Mass Effect (2 and 3, too)
Oblivion (Every single motherfucking Oblivion gate closed)
Skyrim
Assassin's Creed (as well as II, Brotherhood, Revelations, III)
Deus Ex: Human Revolution
Bully

And my personal favorite: Grand Theft Auto IV. That one was a monster.

Edit: Forgot the Kingdom Hearts' (pick one), the Portals, and fuckin' Dragon Age: Origins and DAII.

I stopped caring about this many years ago, but before that time I remember getting 100% in Crash Bandicoot 3, GTA: San Andreas and DragonballZ: Budokai 3 (I spent hours just going in and out of that shop to get the capsules I needed).

I don't do it anymore though, simply don't care.

Off the top of my head, Banjo Kazooie, DK64, and Diddy Kong Racing. Ocarina of Time and Majora's Mask. Half-Life 2 I think also. Those are the main ones I remember the most, though I'm sure there are a couple others.

Boy DK64 was brutal...

Achievement wise..

Call of Duty MW2
Sly 2
Uncharted 1, 2, Golden Abyss
Infamous 1, 2
Jak and Daxter 1
Mass Effect 2

I could also add Heavy Rain and The Walking Dead to the list but that would be kind of cheap.

Trophy wise

Dragons Dogma
Far Cry 3
Darkness 2
Duke Nukem Forever
Sly 1&2
Sleeping Dogs
Rocket Bird (that was a cheap one)
Saints Row 3 (that was also piss easy)
Dead to Rights Retribution
Splatterhouse

Uncharted is pretty close to the platinum trophy and i still have to finish The Walking Dead.
I have also some games with 100% sp complementation like RDR and i mostly skip mp trophies or additional highscore challenges like in the last two Batman games.
I started to like trophies and i always give them a try aslong as it`s not a stupid boring grind ("get level 50 in mp", yeah, sure) or to extreme ("MP3 New York Minute Hardcore" play the whole game in one sitting, do it fast and every shot has to count, no checkpoints, don`t die and don`t let the time run out or it means restart right from the first chapter. I really don`t have time for this).
