Its a well known fact that early experiments into cloning produces results that didn't have nipples or belly buttons. You need to check to see if he has a belly button, otherwise he could be a failed government experiment to genetically engineer clone super soldiers.
If thats not the case then I don't know so I will move on to more serious answers. Does he have areola or is that missing too? Its possible he's had breast cancer and thus had bilateral mastectomies but thats very very unlikely for a male of school age to get breast cancer and to get it on both sides is even rarer.
