I think somebody already mentioned it, but season 2 of Death Note.I thought it fit the show better than the first opening, as it kinda played on Kira's descent into madness.

How did I forget Baccano?! I thought of Lucky Star, but was like eh about it. I only saw one episode of Haibane Renmei and Fate/Zero, and don't remember the OPs. Angel Beats' OP could have been much better, and because of that I find it hard to decide if I like it or not. Forgot about After Story's. Its a good song, but I hate the series quite a bit, so its not something that comes to mind when I think of "good anime."[Kira Must Die said:

I'm currently watching Full Metal Alchemist (I know I'm late to the game) and the intros are extremely underwhelming.
The Black Butler and Soul Eater themes amped me WAY up to watch the episode and got me excited. The FMA theme makes me... well... bored.
Granted I'm not too far into it but the themesong changed once and it's still lame.
By far my favorite, though:
It was one of my favorite animes. I used to watch one episode each morning while I ate my breakfast. It was the best way to start the day.
Hearing that themesong again and not having it continue to the title of the episode and onward really sucks.

I love the openings of Bleach and Naruto, but that's too many to list obviously. "chAngE" by Miwa is one of my favorites for Bleach, and just adore openings 5, 6, and 7 of Naruto Shippuden. They do a great job of getting you excited for the arcs covered.
Gintama's fifth opening, "Donten," by Does is one of my favorite openings, though it's more because the song is catchy.
The ending that uses "Some Like it Hot (Samurai Heart)" by SPYAIR one of my favorite endings. The title is also a terrible pun.
Unfortunately it's all but impossible to find just the opening animations on youtube.

Code Geass and Neon Genesis Evangelion have poor intros for the series in their choice of songs, the styles do not really fit the anime's as far as I am concerned. They are kind of cheesy, and a bit too upbeat for the type of show.
