For me, the MGS series has some of the best and some of the worst plot twists.

Best: MGS4. Right at the end when Big Boss suddenly appears and stops you from killing yourself. For me, that was really unexpected.

Worst: MGS2. Your entire journey has been controlled to replicate the Shadow Moses incident to us as VR training for later soldiers? It's not that it was particularly predictable, it's just so stupid!

Also worth mentioning (for best):
Would you kindly? (Bioshock)
Jade's uncle gets infected with donz virus thingy (Beyond Good and Evil).

A rather interesting one was Dead Space...
with Nicole being dead the whole time, that was kinda obvious. I did however like how the first letter of all 12 missions spelled out NICOLE IS DEAD
New Arrivals
Intensive Care
Course Correction
Obliteration Imminent
Lethal Devotion
Environmental Hazard

Into the Void
Search and Rescue

Dead on Arrival
End of Days
Alternate Solutions
Dead Space .]

I hated that. It's like Valve decided near release that not enough happened in Episode 2 and threw something in at the end.
two Handlers (that's what they're called right?) just barge in and eat a guy's brain then get scared off by a robot that they could imobilize fairly easily.
They're called Advisors. I mean, the ending scene with the striders and the bombs that takes FOR--EVER was "anticlimactic" or something. I guess they had to do something that would make you want to buy Episode three which they better release soon or I will go fucking insane.
But I thought that the missile launch closing the superportal was climatic enough

Worst: World of Warcraft - What?! 10 more levels?! NOOOOOOO!!!!
ahahahahahaha

Best: Jak 2 where Kor, the old guy that helped you...
turned into the Metal head leader, maybe not the best but i didnt exactly see it coming.
And the little kid that is around for most of the game actually is...
Jak himself, taken back into the past by young Samos
Best again: Jak 3, Jak =
Mar

Yes, i liked Jak and Daxter

Baten Kaitos had some great twists!

The voice acting wasn't amazing, but it was worth it for the awesome plot.
Yes, it a twist that bent my mind in half. I don't think that you can even think of a more innovative twist. Seriously, game play and genre defying stuff here.

My exact reaction right there...enlightening.

Megaman 5.

Now keep in mind, this is NES, and plotlines of megaman are as follows: Wily goes batshit bonkers, makes 8 master robots to take over the world. Megaman is sent to keep the idiot in check.

Megaman 5 starts out differently. Dr. Light has been kidnapped, and the napper is....PROTOMAN?! And of course he has 8 master robots that plan to take over the world....etc.

After beating all 8 master robots and fighting through 4 more grueling levels in Protoman's fortress, you comfront him. Only to realise it's an imposter! The real Protoman shows up and give you something to defeat the imposter Darkman. when you think that its all over, it isn't. That bastard Wily shows up and reveals he was behind the entire thing! And then you go to his fortress, 4 even more grueling levels later you defeat him and its finally done. Megaman saves the day!

My god, back in those days...That many plot twists boggled the mind! Capcom writing at its finest!

Wow, read the entire thread and there's no mention of the legendary ending of Fallout 1?
Thank you for saving the world, now if you would kindly turn around and walk away as we've just exiled you. Bye!

Bad:
FFX-2, because she's a...? And he's a...? And now? What? Cue more singing...

Love all the mentions of Baldur's Gate 1 and 2 btw.

Good: I'm going to have to say Legacy of Kain: Soul Reaver 2. Seriously, did anybody see that coming? Time travel = delicious mind-fuck.

Bad: The Jak series. Great games, but the "twists" were very predictable, in my opinion.

Bah, that's nothing compared to the previous game's 8-bit bombshell: Dr. Cossack did NOT go rogue out of mad jealousy of Dr. Light's fame and success. His daughter Kalinka was kidnapped by Dr. Wily, who somehow miraculously survived being crushed by the collapsing ceiling in Megaman 3. He then secretly blackmailed Cossack into using his own robot masters to start a new rebellion.

More seriously, I'd consider some of Castlevania: Portrait of Ruin's twists regarding the Vampire Sisters and the VK Whip to be pretty unexpected, to say nothing of Aria of Sorrow before it. Eric Lecarde has a strange fashion sense regarding his teenage daughters. Soma is Dracula.

Plot twists are always a difficult thing to manage, especially in games. You have to include some kind of foreshadowing or folks complain it's too out of the blue. But some gamers are sharp or experienced enough to pick up on the slightest hints and instantly speculate then complain it's too obvious. I haven't played through Metal Gear Solid 4 yet but I've been accidentally exposed to enough material to tell that it's writers went to the same school as those of MGS2. You can only stack up a maximum of 3 or 4 massive twists before it just gets stupid. All major twists should have some kind of impact on how or why you fight your battles.

Fatal Frame II: Crimson butterfly

Getting to the end of the game to choke your sister to death and throw her in a hole - was not expecting that.

Besides the Fallout 3 (I'm still pissed at that)
One of the most slap-to-your-face-endings was Fable 2
It was real stupid.
