Best Design in a Tabletop Game

Any buddying creators of deadly traps or masterful beasties of the tabletop realm here that wants to share their designs with us?

I'm looking for DM's, GM's, Keepers, whatever, who created their own spells, weapons, puzzles or whatnot for their own scenario. I'm not looking for a character sheet or something that's a rip off of another thing or a design straight from the book, but your own unique thing. Even a random encounter you thought of that was a good load of fun.
Stats and rules wouldbe helpful too if you wouldn't mind other people using it for their games.

Two people I know, one love to make traps and the other, she seems to come up with a few awesome puzzles.

So hit me. What has your been able to muster outside those pages and that square?

I have a weapon:
Gnomish mace, deals 1d6 at Basic level and can reach three feet out through some kind of contraption with a flick of a button. One handed.

Basic mastery level: 1d6, no special things.
Skilled mastery level: 2d4, a +1 bonus to AC.
Expert mastery level: 2d4, +2 bonus to AC, stun, entangle.
Master level: ???
Grandmaster: ???

I don't know the last two, I'm only at expert right now.

So what version of DnD is this used in? Also, I would imagine some kind of Free Action spell would be really handy with it, seeing as Entangle tends to be a duel edged sword sorta thing. It'd be cool to run through the vies clubbing everyone in the knees.

I made a monster onces for CoC, but I never got around to using it, so I'll give it over to you guys:

The Creature

This creature is a frog looking thing, with a dozen legs and over a dozens of black eyes instead of a neck and a head. The eyes tend to come from invisible slits and open whenever it feels like it. Erupting from its shoulders spawn two tendrils with sharp and poisonous hooks on the tips. On the back of its green putrid back is a large scar. When it raises its food above its back, the scar splits open with gooy stings of acid is revealed to be a large gapping maw with rows of spine-like teeth and many inquisitive tounges. The acids in its mouth will desolve any creature it pulls in.

It lurks in the murky swamps and damp places generally shunning the light. Shinning any light into its eyes will force the creature to close a few, but never hurts it.

Damage Bonus: +2d6
Weapons: Tendrils 55%, damage 14d+2d6 (per tendril) plus poison (POT 15) or paralisis overcomes the investigator.
Grapple 45%, damage special. The victim must resist the creatures Str or be lifed into the air and devoured on the next round taking 3d6 points of damage per round thereafter.
Armour: 2-point hide; regenerates 4 hit points per round unless dead.
Spells: None.
Sanity Loss: 1d4/1d8
