probably fighting brayko in alpha protocol and thinking it would be a good idea to fight him hand to hand
and then for some reason not backing up cos the little voice in my head told me i could finish him from about 90% health before he knifed my ass into oblivion

*Mild Super Meat Boy spoilers ahead*
In Super Meat Boy the other day, I was on the very last boss fight with Dr. Fetus (which I found pe-reety hard, by the way) and I died at the very last second- I had already triggered the switch to win, but a rocket nailed me before Dr. Fetus could fall to his doom.
However, I did manage to actually win not long after, and there was much rejoicing.
In Super Meat Boy the other day, I was on the very last boss fight with Dr. Fetus (which I found pe-reety hard, by the way) and I died at the very last second- I had already triggered the switch to win, but a rocket nailed me before Dr. Fetus could fall to his doom.

in supreme commander as i always play defensive for most of the game, so when this happened i had allot of turrets/artillery but didnt have much in the way of combat units.
So im happy building stuff then, flying towards my base comes a aeon flying fortress[1]. i managed to shoot it down just before its main laser hits my base. But as it falls it hits my tech-3 fusion reactor and blows it up, this causes the next reactor to go up, and the next and the next. unfortuantly my command unit was next to the reactors as he was building another one. So he went boom; and if you loose him game over....
so i lost a game to 1 unit falling in the wrong place.
