Ok, putting my choices out here..

Worst: Frozen Frog from DMC4

While not terrible, it is incredibly mediocre compared to all of the other themes within the game.

Best: Either Deep Ambition from RE5 or The Midnight Carnival from Guilty Gear XX

Deep Ambition is a full on orchestral track, which just immerses you into the final fight against Wesker...but eh..I'm going to have to hand it The Midnight Carnival, for being a pretty rocking rock/metal tune, it goes with the boss, and it makes her boss fight that much more awesome and bearable.

No boss music has ever inspired more of a "Oh shit!" response in my opinion:

Not necessarily the best, but my favourite.

I can't think of any shitty boss themes right now.

One of my favorites is Fatalize in Tales of Symphonia. It plays when you fight Sheena, Regal, and Abyssion. I can't think of any bad boss themes, but I'm gonna go with themes that don't fit with the setting or the boss.

Rather intense music for the when Patriarch appears directly next to us and destroys us all with one missile while we're still in the cutscene of him going "RAAAAAAUGH".
