Just finished my first playthrough. Awesome game! Zaeed and Jack died in my suicide misson, everyone else survived.

all my people survived on the first playthrough i had legion do the hacking and garrus lead the squad both times and i had zaeed bring chakwas to the ship and i had samara do the biotic shield

Only two casualties for me.

I chose Tali to go into the shaft, but she got shot while closing the door. I was suprised, because she was 100% loyal. I never liked Tali, but the music made me feel kinda bad for her. First time that ever happened to me.
Zaeed led the first fire team succesfully and Garrus the second.
SuZe held up the biotic barrier.
Grunt escorted the crew back without casualties, aside from Chambers who got liquefied.
Then after the final battle Miranda died. She was the only one who wasn't loyal, so I guess that's why. Then Shepard and Zaeed ran like hell. That was the most epic moment in gaming evar.

And FU to all the Zaeed haters, I thought he was awesome.

I had everybody loyal except jack and legion (I play something of a mix between paragon and renegade so i did not have high enough paragon/renegade score to be able to resolve the arguments whit out picking sides). I choose to abandon the factory workers to get Zaeed loyal because I needed every possible team member loyal to save humanity so sacrificing the factory workers was a price I had to pay (this is also why I did not give the crew any escort back because in the big picture your crew is not worth puting the mission at risk).

Tali sneaked trough the wents and died closing the door.
I let Zaeed led the second team because he had experience leading the blue suns.
Zaeed died after leading the seconded team 2 times.
and jack died while guarding our rear I did not get to see her die but I saw her corpse as I flew away before blowing the place up(fuck you illusive man)

I was fortunate enough to have everyone survive on my first playthrough :D

Omega 4 Relay
Normandy was fully upgraded so there were no casualties on approach to the Collector base.

First part of the mission
Tali and Grunt were part of my squad.
Sent Legion into the heat pipes.
Had Garrus lead the fireteam.
Zaeed took the surviving Normandy crew back to the ship successfully.

Second part of the mission
Tali and Grunt in squad again.
Samara held the biotic barrier successfully.
Garrus, once again, led the second fire team.

Final part of the mission
Yet again, Tali and Grunt in the squad.
Garrus (or was it Miranda?) led the rear guard.


We all survived and I made the wrong decision by letting the Collector base survive. I shall not make that mistake again.

I had Miranda as second team leader, Thane went through the vents and got a face full of gun at the last possible second, which pissed me off. Jack fell at the end of the shield and I lost Garrus, Tali escorted the crew back, Miranda and Jacob came with me to the final fight, with Jack holding the door. I lost two of my favorites and one I hated. Guess who's who. Bonus if you can figure out why that hate doubled.

Reading the bios of your group members helps a lot. Miranda and Garrus are good for the fire team leader for example, because they both have experience leading operations. Picking Thane, Samara or Zaeed might seem like good choices as they all have lots of experience killing stuff, but they lack leadership qualities. And that lack of leadership costs you team members.
see i did this, and i put thane to do the security hack, because thats EXACTLY how you aquire him, and he was like a fucking ghost, which is bullshit he got headshotted closing the door, and i was super tight with him so he should have survived no matter what, but he got annihalated, same with jack, she was a fuckin pussy biotic

Just finished my first play-through and lost 2 members:

Mordin was sent through the pipes and then got a shot to the head. Fuckers, I liked him and his hyped up jabber.

Jack died leading one of the fire-teams. Got shot in the gut and died, should have sent Zaeed instead seeing as he's pretty much cannon fodder.

Zaeed led the crew back and made it...should have chosen someone important instead and sacrificed him.

Samara was my bubble person and did a damn good job of it.

Tali and Legion helped me take down that weird ass reaper and survived along with everyone else who held off the collectors at the doors.

Gonna have to reload my pre omega 4 relay save and save everyone...except Zaeed. He can die seeing as he was just DLC and was pretty expendable. No one else should die as ME2 actually had a cast I mostly LIKED this time around....although sending that ***** Ashley or the Red Shirt Kaiden to their deaths would have been funny but alas that wasn't meant to be. I preserved the collector base, but I'm willing to go back and blow the damn thing up and kill Sheperd. I don't mind starting off ME3 with an entirely new cast if I kill off all these characters, would be interesting although we all know that the vanilla storyline will have him alive.

Exact same thing happened to me, I had Legion infiltrate the base, Jacob led the first fireteam, Thane escorted Chakwas and Co. back to the ship, Jack (Subject Zero) led us through the walkway and kept her biotic barrier up the whole time (she even had enough power left to put some hurt on the Collectors chasing us afterwards), and then I had Miranda lead the defence while Garrus, Mordin, and I killed the last boss.

I'm so glad all of my crew ended up surviving, all of the characters turned out to have likable personalities. (I went paragon mind you.)

Also, if Garrus or Mordin had died, I would have never played the game again. Those guys rocked, and never left my party for the entire game.

Pretty sure I had Thane defend the doorway while Shepard took on the Reaper with Tali and Garrus.

None of the Normandy crew died and only Legion got killed after being swarmed with bugs. I think if I'd chosen a better biotic for the barrier (like Samara or Jack), they wouldn't have stumbled and Legion would have survived. Might have needed Legion's Sniper Rifle as well.
I had that except Jack as the biotic barrier, and many of my squad's arguments were resolved by me being better at shouting. I can't remember who I set to hold the line, possibly Miranda again, took Thane and Mordin to the final battle.

Everyone lived.

If you read the other posts here, you'd see why. You picked the wrong person to lead the Fire Team. You need to pick the correct hacker AND the correct team leader or else someone dies.

Legion:
Lost loyalty due me siding with Tali in argument, sent in to hack security system, panicked and screwed up closing security door, causing it to jam, hit in the face with biotic attack from harbinger while attempting to manually shut security door.

That is all. The second time around everyone will survive.

SHEPARD:
Full paragon, and about 30% renegade.






INFILTRATOR:
Thane. During the pre-mission discussion, Jacob remarked that the vents would be a suicide mission, and I figured Thane was the most expendable teammate; he was dying anyway, and he'd already been preparing for death by making peace with the galaxy. Plus, he was an expert at infiltration (remember how Nassana died?) and preferred arid conditions too. Even though his tech abilities weren't that great, he seemed good enough for the job... right up until his head got blown off by a missile anyway.


1st TEAM LEADER:
Samara. Before the mission, Miranda suggested that I pick someone with excellent leadership, good combat abilities and a wealth of experience. Samara's a 1000-year-old Asari Justicar... if she doesn't fit the bill, who the hell does? Plus she can keep cool and disciplined in a firefight.


ESCORT (to protect the surviving crewmembers en route to the Normandy):
Jacob. Didn't need him for the rest of the mission, and I figured that he was a good choice; he's a well-trained soldier and biotic, and seemed to be the kind of guy that'd protect them, no matter what. I figured that if I sent Jack or Zaeed or someone and the group came under attack, they might just abandon the crewmembers to save themselves...


BIOTIC (to shield the team):
Samara; she's as powerful as a bloody Asari Matriarch, so I figured she'd be able to keep everyone safe with a strong barrier. Plus, I didn't trust anyone else, as Jack seemed to me to be the only other teammember powerful enough for the job, and like before, I figured she might just throw in the towel and leave everyone else to die in self-preservation if things went to hell.


2ND TEAM LEADER:
Miranda. She's apparently an expert at tactical command, and I couldn't use Samara, so she seemed good enough. I was a bit worried about putting her and Jack together though...


FINAL ASSAULT: I took Garrus (a squadmate I could trust) and Jack (who I preffered to keep within eyesight, and not leave with the others) into the base to fight the Reaper. Left Samara in charge of the rearguard force.






END RESULT:
- The Normandy survived the debris field and took out the Collector ship with the upgraded Turian cannon.
- The Infiltrator (Thane) was killed trying to shut the door after it jammed.
- All crewmembers were saved from the harvesting pods, and escorted safely back to the Normany by Jacob.
- Neither fireteam took any casualties, and neither did the rearguard team.
- The Collector base was destroyed.
- Most importantly of all, Shepard survived (plus Joker had an Arnie moment).
