I found the new Castlevania to be really hard on Hard bastard difficulty.

Espcially trying to do the Trials.

Demon Souls on New Game plus is the literal definition of a nightmare. There are quite a few enemies that will kill you in one hit, even if your're at full health, in body form and are wearing heavy armour.

Halo 2 legendary, it might not be as old as you want but it can still be tough. Very tough.

Any Touhou game on Lunatic mode ... if you like going crazy.

BlazBlue on the hardest difficulty setting ... if you like fighting games.

EDIT:

PC games only por favour? I don't own any consoles.
Touhou Lunatic! You will not find a game more difficult than that. The only question is whether is the kind of "difficult" you like.
Blazblue is not that bad. Even on the hardest. The only character that is hard in Blazblue is Hakumen and even he can't hold a candle to some of the other characters in other fighting games.

I Want to be that Guy, Megaman games, SNK fighting games, DoA 4, DMC on hardest, Ninja Gaidens on Hard, Contra and the Double Dragon and BattleToad games.

Mount&Blade: Warband is one of the hardest games I've played in a while. Especially when you use manual blocking and disable quitting without saving.

It's not Nintendo hard; but playing for over a year in-game, building up a mighty force, finally taking a castle for myself, then having the ENTIRE Sarranid army attack it (the fight was about 150 vs 1500) and losing everything... that hurt.

That hurt like a *****.
Eh, now try the new one: With Fire & Sword. This ramped up the difficulty WAY up. EVERY bullcrap unit has a damn musket, instant death all around *g*.

Hm, can't remember playing any game the last years where the standard, only, or medium difficulty was intense. And most "hard" settings are just artificially made harder through cheating, unfair or just ganging up AI.

Shinobi has yet to be mentioned, so I will bring its lack of a floor level design to your attention

Has anyone said Battletoads yet? It would be easier to take over the world whilst threatening the world leaders with a frying pan.

EDIT: Sorry didn't see the last decade part.

How about Global Defence Force on the PS2
I still don't care about the whole last decade thing. I swear to god that battletoads is unbeatable (I've been playing it and King of Dragons the last few days) I cant get past the 3rd real level (no I don't count the checker board as a level since it's meant to give you free lives) Got to the fucking jet-ski like level and I just can't beat it. I had over 9 lives and I swear to god that it decided that I was doing too good so it decided to move one ramp on me.

And if I remember correctly (seeing as how I was like 5 or 7 when I first played it) There should be ANOTHER jet ski level
