Well I used to rp on internet forums alot but I don't really do that anymore, but because of that I have LOTS of left over names that I use.

However, Vohn Exel has always been "me" whenever I use him, so he's my first name just about anywhere. Whenever I want to make another name, I usually just reverse mine to make Lexe Nhov (usually for evil characters, he's the opposite of Vohn...it's a long story.) But I have about sixteen other names that I cycle through just because.

Since I made a fucking awesome character on Morrowind I've used this guy for every game that I can name the Protagonist, Marcus Octavius.

Although if we're Morrowinding this shit up, which we are now I've had loads of characters names.
Mufasa(Yes, he was a Khajiit), Esmeralda, Sebastian, Marek, Arcanir, Mordrid and billions more.

My name for anything single player, MizPiz or TheIr0nMike for any online community. I really haven't gotten into any tabletop or online rpgs so I haven't given myself a fantasy name.

Any form of Dark Wizard / Warlock / Necromancer: Xeros.

Any Rogue, or knife/stealthy type: Sylis.

Any mountain-sized warrior holding a sword the size of a small car: Atalis.

(Atalis is also the name of any shamanistic type characters.)

When I play Legend of Zelda, I always go with Link. It fits the story best. When I played RuneScape, my character's name was Macgyvercas (which is where my username here comes from). In DDO (which I currently play) I have four characters: a Human Paladin named Drakelius Des Morte, a Human Wizard named Ganondorff Dragmire, an Elf Ranger named Neebs Dorendolion Aldavathar, and a Human Cleric named Redwald Firestone.

Almost always senorfatso, in an ironic attack on my borderline anorexia. Sometimes Pewpotamus or a derivative of such.
When faced with naming an entire party, I usually resort to using the names of the members of Public Enemy, then any Rhymesayers rappers I can think of. Or Maurice/Jose, because they are what I wish my real name was.

For most of my characters I name my characters Gorlock.

It sometimes doesn't fit the character but I still like it.

Let's see, I usually use Kryzantine, with alts having a few modifications near or at the last letter. But my most successful WoW character was actually named Maõri, and unfortunately, I couldn't get a straight bar over the O. Apparently, every Aussie I met recognized that limitation and congratulated me anyway, leading to about 5 private msgs every day from random bypassers asking, "Are you a New Zealander?" or "Are you an Aussie too?", after which I had a lot of fun explaining to them I was a Russian-American living in a Jewish ghetto in NYC. They usually followed up with, "But... you know about the Maori... you even have the special character over it...". He was a Tauren Shammy, fittingly enough.

In JRPGs I usually just leave the default character name but I do have a few standards for other games.

In Pokemon, my character is always named Golden (Gold was the first Pokemon I owned) and my rival is always Luke (my best friend). Alternatively, my sister always names her character after her and her rival after me.

In most fantasy games (I did it in Fallout 3, Dragon Age, and it started in Rappelz when I played that years ago) my male character is named Altheron and my female character is named Aneira.

Garritios normally, Tai Kaurgi if it is a samurai game, Wang Pu otherwise.

The 1st 2 sound cool, the last one is just a funny name I came up with while making a Dynasty Warriors character (who wields a stupidly large sword most of the time).
