I don't remember ever seeing a fat Helghast, but yeah, that would be pretty cool, although probably about as hard to make as a Black Hand (the gas mask and all the tubes). And I thought I had read somewhere that it was because Fatman was too fat to walk, but there's a good chance I'm wrong.
What about a Hunter (Halo)? That would be widely recognized.
