A particularly sad and creepy one in Arkham City is Harley's pregnancy test. Finding it before the HQR dlc and you can see the result is positive. After the DLC, however, the result changed. Which can either mean a false positive, a miscarriage or that she had an abortion.
I hope that they make Harley the main antagonist in the next Arkham game; I just think that she's more interesting than the Joker [flameshield] because, think about it, she must have been sane at one point. It's implied that she's had an interest in villians for a while, but I'd like to know more about her.

My one is OFF. The entire game. To elaborate, after you finish a zone (read: level) after beating the gaurdian {read: caretaker and runner of said zone, functions as the game's bosses) and if you return from the Nothingness (read: The ethereal hyperdimension in which the batter {read: the ethereal Jesus Christ of Oz {read: the game's setting, sort of}}} then... well... it's monochrome, and you get attacked by demon. Babies.

GAH! GO AWAY, YOU AND YOUR STATUS-INFLICTING SPELLS, YOU LITTLE CREEPY...

captcha: industrial revolution. How odd, OFF is basically a game about industrial revolution gone wrong. Thanks, France, you made me scared of children.

Fallout 3 and New Vegas had a lot of this...

In Point Lookout, a grown skeleton, a cradle with a baby skeleton in it, the grown skeleton holding a gun...put it together.

Also, I have seen two skeletons holding each other, a gun in one of their hands.
I think the skeleton with a baby is a crude joke about a mother/daughter suicide I'd heard about, way back in the 90s. I can't remember the names, but it was on the FBI Files, if you really want to track it down. The mother had shot her baby daughter and then herself with a 8 inch magnum. That just makes it more disturbing.

Pretty interesting, though, how they got away with that in a game that was really heavilly censored in a lot of places.

She first appeared in Batman The Animated Series in the 90s, and there's an episode called Mad Love where they give her a backstory. Basically she used to be a criminal psychologist in Arkham, she was fascinated by the criminal mind, but she fell in love with the Joker, who persuaded her to break him out. As Batman says "She's just another one of his victims".

uh... yea... those 'demon babies'... not only are they creepy as all f-ck but just the thought of where they came from... the Maiden sits up on a rock in the back edge of the pool of 'blood' those horrid things spawn from... and her used to be clean white dress is bloody... only not just the tips dunked in the bloody water mind you... no... no if you look hard enough you can see it it goes 'all the way up'... then you remember the line 'after learning the truth, the Sixth Saint Astraea gave herself over to the deamon'... there's honestly only one way my brain can interpret that... and being female... it makes it extra squicky for me o_O;; demon-rape, demon-babies... and by the gods Sir Garlan has to sit there with her and watch it all... *twitching*

... now i remember why i love Demon's Souls over Dark Souls... despite all the 'unsaid' its really disturbing when you think about it...
I don't recognize that moment and I have all the achievements for the game. Where in the game is it from?

... its Maiden Astraea, the final boss of the Valley of Defilement, if you kill Sir Garlen first, you can literally walk right up to Astraea, she won't do anything except basically tell you to kill her, you can get a good look at her, the dress, and depending which way you went to get to her have to walk through the Demon-Plague baby pool...

as for the quote, its part of her backstory thing, and i believe one of the NPCs in the Nexus or in VoD says it as well, i know Garlen's sister makes this big deal about all that cause she's angry at her brother for following Astraea still...

you can get all the 'trohpies' for Demon's Souls easy, they're only dealing with crafting and beating the demons, not on all the dialogue or small details... i believe the Pure Blood Sword has mention of all this too... it requires looking deep into... and like i said, DEMON'S SOUL not Dark Soul

Easy to miss given that you only mention which one you're talking about right at the end of the post, 'cause you confused the crap out of me.

In one of the story paths in The Witcher 2 there is a scene were a king has a sorceresses' eyes scooped out with spoons. It happens off camera but there is audio and you have a conversation with the now eyeless person right afterwards. My day could have done without that.

The Hawaii sub plot in Hitman Absolution, from when you go to the strip club and kill Don Osmond. Its interesting how they set up the level so that you're constantly getting hints as to how fucked up the entire scenario is. I mean I was honestly horrified when I finally found the chair, and kinda glad I got to garrote the guy responsible.

I don't think it really compares to anything else in this thread, but I do know one kinda gross thing about Gears of War.

You know the Seeders that release nemacysts into the air? That's not their mouth they're shooting them out of.

Isn't this the whole world-building in Bethesda games? Fallout 3 and Skyrim are #1 and #3 in my top games respectively because of things like that.
Also I remember in Dark Souls there was a place I think called Anor Londo that was a dark and abandoned city. I can't remember why it was that way, but I remember the reason was very gripping and chilling. Obviously not enough to make it memorable though :L
Also read the planet descriptions in the Mass Effect games. Same thing there.
Anor Londo was the capital city of a race of demi-gods known as the Lords, who ruled the land of Lordran. The leader of their pantheon, Gwyn had three children, Gwynevere, a goddess of fertility, Gwyndolin, his youngest son and a master of moon magic, and the unnamed elder brother who was banished. Eventually the first flame (a mcuffin which essentially empowered the Lords to rule) began to fade and Gwyn left on a pilgrimage to kindle it. He reached the kiln of the first flame and threw himself into the center of the inferno, kindling it with his powerful soul. This stretched the time of the Lords a little further. Eventually it began to fade again, and with no god to sacrifice himself, the world began to grow dark. Gradually the other Lords left Anor Londo, perhaps for another plane of existence. Eventually the frail Gwyndolin was the only Lord left, keeping vigil at the symbolic grave of his father. In order to fool the humans of Lordran into believing that the Lords still ruled, he created the illusion of Gwynevere to publicly rule Anor Londo, as well as an illusion of the sun to light the darkened city.

Goddamn I love Dark Souls Lore.

As for disturbing moments? Dark Souls has you covered. My personal favorite:

The duke's archives, the worlds largest collection of knowledge presides over by an insane Dragon Duke, Seath the Scaless. Seath was an outcast among the dragons, born without the scales of immortality, he betrayed them and aided Gwyn in hunting them down. For his treachery he was awarded dukedom and a tower in which to carry out his research into immortality. He was known for having his servants kidnap maidens to experiment on. While the player is exploring the prison tower you come upon a cage full of hideous monsters who are all hostile except for two huddling in a corner at the back. If you get close you can hear the sound of weeping. if you attack them they won't defend themselves and when they die they both drop miracle scrolls described as having been given to Gwynevere's handmaidens. Seath was often reviled, even among his allies, this is just one reason why.

I love this theory and it IS creepy as fuck, however I kinda think it's debunked by the descriptions from the valley of defilement archstones. It mentions that the swamp is a dumping ground for unwanted babies and the sick and twisted, So i'm pretty sure that the plague babies are just demons that have taken the souls of said babies. Also I'm pretty sure that the whole "gave herself over to demons" just means that she consumed and utilized demon souls, much like how other characters mention that using demon souls makes you a tool of the demons. But hey, I like your theory too, but maybe it's just because the whole demon-sex thing weirds me out or maybe it's just because I totally ship Maiden Astraea and Garl Vinland, but I can't quite get on board with that one.

Tons of this sort of thing in Metro 2033, in particular there are a lot of highly detailed corpses, also there's the death cries and animations in Red Orchestra 2.

I have a few of these moments, but there a bit more out there then toaster in the bath tub or suicide pacts. Two really stuck with me in the end

1- Recettear - This is a game where your run an item shop.... Dark, right?... but at one point it makes a God Father reference of waking up to a horse head in your bed. That and the fact that your father abandoned you with massive debt, and if you fail to overcome your debt, your friend repossesses your house and renders you homeless. As someone who has had to deal with poverty, it really hit home in a hard way.
2- Portal 2- DaMaGepy mod maps for steam workshop. In this set of maps, most of the music is left out, as are all the silly voice overs from Cave Johnson, GLaDOS, and Wheatly are also absent. You are left with ambient noise from the game engine; Dripping water, moving platforms, lazer sounds, and the turrets. Without the humor, you are basically in an unknown location, with cameras watching your every move, completely inescapable. In the context of the game, you don't know why they are there, why you are their, what you are doing and where it all leads. It give the game a really creepy atmosphere, making you feel totally isolated, but never alone. At times it feels like theres someone breathing softly on your shoulder.... never thought i could see portal as anything but funny and entertaining.

The fact that Opoona required you to sacrifice 3 of your best friends at the end, and the main purpose of making friends with the characters in that game was to sacrifice 3 of them to progress to the end boss, was quite disturbing, even more so considering children could partake in this.

In Saints Row IV, the cop cars are all 2-seater Ferrari knockoffs. This tells us 3 things:
1. They're faster than most cars, which shows how seriously Zinyak takes the Saints threat.
2. They're also luxury cars, showing that collaborators get better treatment than other people (it's shown that not everyone in the Simulation is one of Zinyak's programs)
3. They don't have back seats; cops under the Zin regime don't bother arresting suspects.

A little off topic maybe, but:

I'm playing Demon Souls right now, and Im all the way into the big temple with the snake head guys, and this ball puzzle thing where you rotate it around and w/e and...

I know absolutely fuck-all about the story?

I mean literally nothing. 0%. I've refused to use any guides, where did you guys all come up with this story? I have no clue as to why I've killed any of those demons, the skeleton dragon, that giant ass wolf, or wtf that huge dinosaur thing is that magically appeared back near the start.

^Isn't that Dark Souls?

Anyway, item descriptions, and small snatches of dialogue from NPCs, they are there for you to piece the story together yourself. That, or watch the Lore videos by Epicnamebro on youtube.

First off, you're playing Dark Souls, not Demons' Souls. Nitpicky, but meh.

Secondly, we all got this story from talking to NPCs, reading item descriptions (which are the backbone of this game's story), checking the locations of items and drawing inferences from them (for example, the location of the Hornet Ring behind Artorias' grave implies Ciaran's feelings for Artorias), and just communicating our theories to each other. Admittedly, you missed a lot of the community's discussions, but there's still plenty of info out there to find and talk about. And, of course, plenty for you to find in the world, to which end I wish you happy hunting! =)

On topic, though, I've got a rather interesting one from Dark Souls: The Painted World of Ariamis. This is an optional area in the game that you need a special item to enter--the Peculiar Doll--and this world contains all manner of things the gods either feared or didn't want around. Case in point is Priscilla, who entered the world via the:

Peculiar Doll
A strange doll in strange dress.
There once was an abomination who had no place in this world. She clutched this doll tightly, and eventually was drawn into a cold and lonely painted world.
This 'abomination' is Priscilla, the Crossbreed--half dragon, half goddess(?)--and she wields the power of Lifehunt, which was feared by even the gods (as if being half-dragon and surrounded by the slayers of dragons wasn't enough). As a child, it seems that she exiled herself into the Painted World to escape persecution, or because she simply felt that she didn't belong with any of the gods and goddesses. When you encounter her at the end of this world, she asks that you leave this '[peaceful land]' and that the 'inhabitants [are] kind'. Who are these kind inhabitants, you ask? Oh, just a bunch of hollows, mutated soldiers, skeletons strapped to chariot wheels, hollows whose bodies are bloated with toxic fluids, and humans who are mutated into birdlike creatures, serving the Goddess of Sin, Velka. That's the company she keeps, along with the large number of bodies impaled on spikes (which still bleed when you hit them). What's saddest of all is that she considers herself better off here than outside, and who knows how long she's locked herself away for.

What makes it a bit more disturbing is that it's heavily implied that Seath--presumed to be the only known dragon alive--was involved with her creation somehow. It's been thought that either Gwynevere is the mother or one of her handmaidens, but I'm of a different mind. The cthulhu monsters in the Archives are his mistakes, but what if Priscilla was his success? Both the Picasas and Priscilla share a crystal breath attack, which Seath is known for, and he was trying to replicate the scales of immortality. What if Priscilla was born from an experiment on the daughter of one of Gwynevere's handmaidens?

So that would mean Priscilla was once normal, but was experimented on, persecuted to the point of her exiling herself in a world for abominations, and now sees these monsters who will kill you on sight as 'kind.' That's pretty fucked up! Sorry this went on so long, but I really love the lore in Dark Souls, especially regarding the Painted World, haha!

Pretty much everything that is in Majora's Mask. Seriously, that game gave me nightmares as a kid.

Failing that, the moments in Metro 2033 and Last Light when you are running from the mutants, and you're just sprinting away and using your flashlight. That first part in the subway in 2033 actually genuinely scared me, especially when that guy got the absolute shit torn out of him. Games dark...

Huge shoutout to Psychonauts too, Mia, Sasha and Coach Oleander in particular. I think someone already mentioned Mia, but Sasha backstory isn't much better. And the child abuse things with Coach Oleander doesn't exactly fill me with joy either. And this is a game with GIANT LUNGFISH!

Fable. Demon. Doors. Seriously half of them are filled with skeletons. Torture equipment, Or worse. But two that stand out are terry cotters cry and the one just before bloodstone. Where you walk in to a pretty snow screen with lamps and a house. All nice an quaint until you step in the house and thee whole place shifts to a run Down shack full of skeletons and discarded weapons. And the lamps at now cages filled with bodies. What the fuck?
