Oh yeah, in Kingdom Hearts, where X is attack, and every other game under the sun where it's jump used to get me all the time.
Jump is X in Final Mix

Speaking of which I played Final Mix to death and then wanted to compare my Sora to my regular KH2 and i could barely select "Load" on the start screen

Only the whole RB vs X to reload/throw granades thing.

Happens between GoW/CoD transactions...

Nope never, I don't even go through some 30 second adjustment time. I just jump right into the game and my brain does a good job at accessing the instruction Manuel right off the bat :p

Fallout 3's control scheme tends to throw me off when I move to games like Call of Duty, but other than that I don't have much trouble with this.

I always had the hardest time with the Metal Gear Solid series whenever I switch games (something I do often). While a lot of games use "X" as the action button, getting back into MGS usually entails 10 minutes of, "No, 'X' cancels out of menus, 'O' opens menus."

The only other hard time I experience is with RPGs. I've done this more than I'd like to admit, but I'll play a few hours of an RPG, switch to a game I just bought or wanted to play again and by the time I get back to the RPG, I forgot everything that has happened (which more or less results in me starting over).

Yeah, I tend to get confused a bit.
One game has a sprint button where another game has the grenade button and some just don't have a sprint button at all. :p
I'd play something like Boarderlands or Call of Duty and then I'd play Halo or L4D, wishing I could run faster and wondering where the sprint button is.

I do get cancel/confirm confusion when switching between say my DSi and my 360 and sit there wondering why nothing is happening cos I in my head I think I have pressed A!

It happened to me today. I switched between Dragon Age and Modern Warfare 2. The differences are drastic. XD

It really only happens to me when I play a game for such a long time I find it difficult to adjust to a different style of gameplay on another game.

haha i totally have trouble switching. i was playing Borderlands until i got Modern Warfare 2. So i switched to MW2 and I kept trying to throw my Bloodwing from Borderlands, and flashbangs I ended up throwing blinded me half the time, resulting in plenty of deaths......

Yup. I played Halo 3: ODST with a friend a couple of weeks ago, after a solid week playing Borderlands. The result was a little ugly to watch.

I've gotten into a habit lately of switching between games, and I find myself getting confused by control schemes sometimes.

For example. RB is the run button in Saints Row 2, but in Gears 2 it reloads, and it Borderlands it throws grenades. I can't count how many times I've thrown grenades when I've wanted to run :).

Does this happen to anyone else? Are there any really bad button confusion combos (like RB for grenades or running) that you can think of?
the main problem I have is when I've been playing something like San Andreas, and then try to play Grand Turismo 4, the driving is so different it really messes me up for a while lol

Transition from Halo 3 and other games to Bioshock. Y=Jump?! X=Hack/Reload/EVE Hypo! RB and LB to cycle through weapons, B to use a health kit etc. I was messing up wasting hypos and health kits and switching from my guns to my wrench by mistake almost halfway through the medical pavilion...although I haven't played Bioshcok in over a year since I beat it so...

Yes, for the simple reason that I play a single game for weeks and then switch to a different game.

It's proving VERY difficult to lose the Fallout3 controls though. Maybe I should use my keyboard a bit more.(there's not a single console port on my pc that I'm not playing with my wired 360 controller)

Yes. I switch between games a lot, particularly if I'm just doing achievement playthroughs and need to avoid boredom, which is bad because once I get used to the Assassin's Creed horsey-steering, I end up crashing my Borderlands car into a cliff, then I have to adjust to the controls in Prototype which bite me in the arse when I want to remember how to jump in Fallout 3. It's my own fault really for being so schizophrenic with my games.

I don't really change games that fast to confuse controls but it sometimes happens to me. Halo 2 controls over 3 made me throw equipment when I meant to reload. Haha crazy Bungie.
