My girlfriend was fired from a coffee-shop for following the orders of her supervisor. She was told to clean a filter out and replace it, given the exact instructions of using a brush to clean out any remaining follicles out of the metal pan then replace the filter with another one that are stored in the blue box marked filters in a drawer. She did exactly that but the supervisor had given her the wrong instructions, she was supposed to use the filters in a yellow box in a cupboard.
This resulted in 3 customers reporting that their coffee tasted wrong and my girlfriend was blamed for it. The boss said those 3 customers were her "three strikes" and she "should have known better and sought confirmation about the filters". Since it was before her three month pre-permanent approval was in place she couldn't contest the dismissal.
I found is strange that she managed to accumulate 3 'strikes' when she wasn't even aware of receiving the first one for following the instructions of her direct supervisor.
