Once. When I rented from Blockbuster. I expected to get Chronicles of Riddick:Assault on Dark Athena. Instead they gave me farcry 2. I played Farcry for a little while then went back to the store and played Riddick. Riddick beat Farcry by a long shot.

I bought a copy of StarCraft and ended up with Warcraft III. I still had the StarCraft license code, however, so I stuck it on b.net and ended up with two free games.
Unfortunately, I lost that account, and the discs. I still own StarCraft, but Warcraft III is gone.

Well, one Christmas my parents bought me Pokemon Gold and my older brother Pokemon Silver. He opens his box, gets the game out and starts playing. I open mine.........AND THERE'S NO GAME INSIDE!! I had to wait like 2-4 days for the shops open again. 
Two years later my parents bought me Ratchet and Clank 3 for Christmas. I open the case, and the disc for Ratchet and Clank 2 is there! Again, had to wait 2-4 days for the shop to open.
It was the same shop both times as well. That's what we get for shopping at K-mart i guess.....

I once bought a Submarine game for the PC. What I actually got was something that just installs then craps itself and dies. Happened with 5 copies of the game. I was less than pleased. It wasn't even the PC's fault. D:

I once ordered a used copy of Mighty Morphin Power Rangers for the Genesis (I collect these things), and ended up getting VR Troopers instead.
It was kind of funny, though, since they're both games based on American live-action shows that adapt footage from Japanese shows.

This isn't exactly the same, buy back in my N64 days I often bought a game, went home, only to realize that the box was empty. There was never any problems with the gamestore giving me a new copy though. When I think about it, that is a really scam-able system.

This never happened to me but quick story. I used to work in a video rental store, and the girl i was working with was making the late calls and left a message on someones voicemail that their son had rented Orgy party and it was 3 days late, then hung up the phone. At this point i came running across the store shouting "Noooo it's called OGRE party!! OGRE party!!"
