I only use mods in Bethesda games, but as far as those go;
Oblivion
-Natural Environments (Greatly enhances the weather system and looks, it's a great graphical mod.)
-Depth of Field Mod (Creates a scaleable depth of field effect)

Fallout 3
-Sprint mod (This is a totally awesome mod that lets you sprint in Fallout 3. It's done really well and enhances play alot. They even added some sprint perks to upgrade it!)

I'm not so much a fan of over-the-top mods, just the casual things that enhance the game while keeping it as the creators intended. This is just a small segment of all the mods I use though. :)

This one for Left 4 Dead:
Brilliance. XD

The only mods I've ever played with were in Sims 2. I found one that was the Necronomicon. It allowed you to set other Sims on fire, put them to sleep, etc., and it always caused the user to go insane. I thought it was cute.

My favourite is OOO (Oscuro's Oblivion Overhaul) for (suprise!) Oblivion. It fixes the problems with the level-scaling, makes the game harder, improves replay value, and generally makes the game better.

One of my favorite mods was Doom Z for Doom 1. It added a whole lot of new Dragonball themed weapons and a DB HUD to the game. After experiencing this I found my own way into modding and level design.

Hmm, Francesco's mod and Less Annoying Magic Experience(LAME) for Oblivion made the game perfectly playable as a mage while making certain items a lot rarer.(e.g. an option to change magical weapon spawnrates, from a 5% to a 100% chance)

SMOD and all it's mutant children for Half Life 2 is great too. The original SMOD added the first flamethrower to the game, other SMODs changed the original to weirder things.
SMOD: Outbreak which is basically Left 4 Dead years before Left 4 Dead was released.
SMOD: Tactical makes the game more difficult, on the original setting any combine with a handgun would kill you in a single shot and they never miss.
SMOD: CSS Sci-Fi changes the original game to twenty objective based missions, from capturing a modified sentry gun to getting an experimental weapon(gravity gun) or just a simple hostage rescue.

Fallout 3 also has some great mods, but two are IMO must-have mods.

Warhammer 40k Space marine armours.
Warhammer 40k boltgun.(fallout3nexus has it as "bolter")

I've found a cool mod used in COD 4 on PC. It replaces the normal guns with paintball guns. It's really well built and adds 2 or 3 new levels. Includes all new weapons, custom classes and new challenges for the guns to unlock firing modes and stuff.

I like it. :p
