The only thing there that is vaguely realistic is Red Orchestra. DoD has lots of OHKs, that's about it. Battlefield gives players borderline Halo health. Others I'm not too familiar with.
Far Cry 2 is more-or-less realistic, so I'll go with that. Also Gran Turismo 4 if it counts.
EDIT: Okay yeah, Far Cry 2 is hardly realistic. But I haven't played a decent "Realistic" FPS yet. ARMA II had like a million controls that I couldn't begin to wrap my head around and the voice acting was horrible.
I don't really play realistic games. Realism means you die quickly, meaning I get bored quickly.
I suppose the only one I can think of I like is Left 4 Dead 2's realism. I mean, it might be just like a real zombie apocalypse. We haven't had one, so you couldn't say it's not like one.
That or Call of Duty 4. Because everyone knows sniper rifles are really meant to be used at close range.
Ghost Recon was pretty realistic for its time. I quite liked it, even trough the game was pretty damn hard. One or two bullets would kill you at the start of the game. You get hit in the leg, you can't run. You get hit in the arm, you can't shoot accurately. You get a bullet in the face: you die. Fun times.

Arma 2 is one, The STALKER games were pretty realistic as well. But I never liked those types of games...I want to be able to tape health kits to my chest to heal wounds and carry 50 guns that all go boom.
I liked the little bits in Metro 2033 with the gasmask watch-timer, the flashlight charger, the journal menu that was an in-game clipboard w/ compass and lighter. Dunno if I'd call it realistic though, but somewhat immersive or whatever enough to pass off as realism I guess.
