Closest I've probably came would be the Mannequin place in Condemned 1. But it's still just a game, just like scary movies are just movies.

Media doesn't scare me, but I love it anyway.

Oh shit, I think I remember that!

Pretty sure I remember going down a hallway in that game and heard some really deep guttural growl. I was CONVINCED a giant monster was about to attack me.

I actually said something like "wasn't there three manniquins minute ago" before I was attacked by one of them. I loved the way the submachine gun was probably the best gun, but if you got jumpy you wasted all the bullets.

The bear in the second game scared the shit out of me. But also there was a great sort of inversion in the hunting lodge, where I became the made hobbo killer slinking the shadows wielding a reindeer antler leaping out at SWAT members. I had become the thing I had feared in the first game, it was awesome.

Also in MW2, the bit where you have to run without a gun away from an angry mob I was muttering "oh shit, oh shit, oh shit". Not because the game was scary, but more the idea was, the crowd catching and tearing apart my defenseless characters and dragging his corpse behind a truck or something seemed so possible.

There are three games that have scared me throughout my career in gaming. They are (with rating):

~Silent Hill 2 (6 out of 10) : This will always be the king of games for me, that have the best atmosphere to make me want to empty my bowels.

~That one Haunted Castle secret mission in an early Medal of Honor game (8 out of 10) : BEWARE THE DANCING DOGS!!!!!!11

~And finally (100 out of 10):

EMERALD WEAPON from FF7!

No bullshit! There I was, cruising along underwater with nothing on my mind, just chilling to find what things I can screw with next, and then BAM! This giant fucking thing comes out of nowhere, heading straight at me at 1,000,000,000,00 BA ZILLION MPH and here I am in my piddly little submarine trying to escape! Then it gets me into battle, fucks me over, and I have been afraid to go underwater in any game ever since. Even the fish levels from Super Mario Bros. is malicious!

To be fair though, FF7 had a bunch of moments that at least made the hairs on theb ack of my neck stand up. Examples being when Jenova breaks out (or anything with Jenova for that matter), and the Hautned Mansion at the Gold Saucer. ^^

And especially, for my fellow FF7 ultra geeks. Who has goen into the dbug room and done the few creepy-ass things in there? Remember, this IS Hades. ^^

EDIT: I'd like to add the ShinRa Mansion to the list. The laboratory down there invokes bad memories...

-The Reaper in Persona 3, particularly when some other voice clip would cover your mission control warning you, so your merry band is just roaming around and suddenly- *jingle* "OH FUCK, RUN! RUN!"

-The Pirates in Disgaea 2's Item World, because I knew I was either in for a great fight or a horrible, painful curbstomping.

-I've been ninja'ed on Emerald Weapon, so I'll leave it at that.

In MGS3, the first time I went though The Sorrow's "boss fight" if you could call it that. The music and all that combined with us not knowing WTF to do at the time (plus us killing EVERYTHING up to that point made things a lot tougher).
That was a long time ago though, since then nothing has done more than startle me. I did enjoy FEAR 2's attempts to scare me, those were the funny parts of it. I actually was always looking forward to the next one.

Silent Hill 2, for the fact i felt like i was always being watched, and the scene where Pyrimid head tossed me off the roof made me yell.
