Well, it's kinda dark, but this happened to me a while back. I was the one driving the mongoose. The rider shot a rocket at the worst possible time.

Earlier today, a freind was over trying out Saints Row 2 while I had it on rental. He started out by giving his female (for some reason, he always plays as a girl in games) the cockney male voice. The real fun started, however, when we got to the first mission to save your buddy at the courthouse (It's the first mission, so it didn't seem neccesary to put it in spoilers...). At the beggining of the cutscene where you go into the court, he yelled "Objection!", Phoneix Wright style. He yelled it again when the protaginist actually enters the courtroom and continually yelled "Objection!" for every kill he made with a shotgun. Based on that combined with what he's told me of the beta, when he gets CoD 5 tommorow, he'll go directly to Xbox LIVE, create a shotgun class, and begin shouting "Objection!" into his mike like a maniac.

Plus I drove him mad by playing the Galaxy Man theme from Mega Man 9 throught his time with the game. :p

A friend of mine was playing Fallout 3 and he VATS'd a Frag grenade to a raider from about 100 yards away.
The raider became mulch and his Assault Rifle flew.
We watched the gun as it defied physics and flew directly at my friend's character.
From a football field's length, he caught the grenaded weapon.
We had a good laugh and a pondering of the odds of that happening again.

Well I've played Team Fortress 2 (on 360) when the gravity suffered a glitch and made it possible to leap from one end of 2fort to the other.
Of course then it was also possible for engineers to place sentry guns on the roofs.

I was playing gears of war with a friend and we were on the train. We had just gotten to the berserker and we were wondering where it had gone(somehow we lost track of the behemoth). And so I turn around and BOOM! I'm on the ground scared and laughing while my friend starts to freak out and just goes nuts on the thing.

My friend had screwed up some of his WoW folders and asked me to send him a copy of the ones he messed up so he didn't have to download the entire thing. So I replaced the music that plays at the logic screen with an extremely loud Rickroll before I sent them to him and asked him to get on vent. About 5 minutes later as he's talking the song starts playing so loud I can hear it over vent and my friend went absolutely nuts. Completely immature and stupid I know but in my defense it's the only time I've Rickrolled someone and the opportunity was priceless.

Had a couple good moments last time I re-played HL2...the ones I can remember:

I was fighting a far-off Combine and noticed an explosive barrel behind him, so I shot it a couple times, setting it on fire. He just kept shooting me for a couple seconds, then stopped, and turned to look at the burning barrel...a moment after he saw it, it went off and blasted him far off the ledge he was standing on. You could almost hear him thinking "...Oh F-" when he saw the burning barrel. (Just as funny is the way Combine snipers sometimes yell "SH%&!" [uncharacteristically clearly] if you toss a grenade into their hideout.)

While leading some rebels around, one of them found a shotgun, picked it up, and just said "Finally" in a dramatic voice while holding it up to load it just as dramatically. It was strangely hilarious watching a jumpsuited civilian rebel soldier suddenly act like Arnold Schwarzenegger in a cheesy zombie movie.

One word comes to mind: Toribash. Not many games do they offer you the opportunity to beat the sh*t out of your opponent with his own leg...

One time during a game of the original smash brothers a friend went for a piss, so we changed all our players to super hard AIs and proceed to obliterate him 12 games in a row.

Online flying high in a helicopter in Battlefield, decided to eject to invade a structure but when I hit the ground I couldn't find where the helicopter had crashed. 5 seconds later the flaming metal heap fell straight on me.

Funniest moment for me was my first (and still only) go on a Halo game (the second).
I was facing my friend, a self-confessed addict who hadn't been killed by anyone in a one-on-one match in something like 6 months. So, me being a rookie, it should have been a slaughter. I managed to get a rifle and sniped him - one shot one kill - from the other side of the map. My first shot as well.

Far Cry 2, fighting at a safehouse. I managed to get near the house then eliminate two of the three guards by it. The third was standing in the road by a small hill. A jeep passed by and saw the fighting, decided to join in. The jeep went up the hill at a slight angle, before flipping over and crushing the third guard underneath it. Ownage.

GTA4. I'm driving at high speed to escape the police, and I crash my car. I'm catapulted through the windscreen . . . . . and out of the cops' search radius. My wanted stars disappeared before I stopped rolling.

One way to avoid arrest, I suppose.

Because is the internet, and the escapist is populated mostly type with the same care as they might were they writing a magazine article or their doctoral thesis. And since we put so much care into our posts, it is somewhat annoying when people don't bother to use proper grammar and punctuation. And with some people, like me, it throws our brain out of gear, like a computer trying to process code that doesn't use the proper syntax. So if you'd like for us to think of you as a regular guy, please type properly, like the rest of us do. It's not exactly trying and you won't get any shit over it.

On to the topic at hand. one of my most enjoyable moments was trying to write my name in limbs pinned to the wall with the Railroad gun in Fallout 3. Didn't work out the way I wanted it to.
