Halo 3, mainly because of the 8 year olds who think there really awsome and that there the best player ever.
In Juiced (the first one) on PC when im trying to win a race (with a very big bet down) and the asshole AI drive me into a wall/spin me out/use "im faster on last lap" crap and cause me to loose. Also i get penalized respect points for them driving into me.
And when people play that are very obviously hacking and nobody gives a shit (especially if they on there team)

Superman 64 (or whatever the hell it is called). I pulled it out of the ol' game drawer when I saw a review on it. I said "It can't be that bad. Those people are just overreacting." Ten minutes later, I had said about twenty words that could pertain to intercourse with someone's mother, about thirty words that could pertain to feces, and roughly fifty words that could pertain to divine condemnation or the male offspring of a female dog.

OT: Most often games make me really angry when I start losing and don't know how to win. I'm perfectly okay with comprehending how to win but not being able to pull it off. It's making a game so agonisingly difficult to win that it seems impossible that really bugs me.
The only thing that's ever bothered me is when there's a time limit for something difficult. It's gotta be something really annoying though, like playing Mile High Club on Veteran difficulty. That pissed me off.
Overall though, even that, which is at my angriest, I've never broken anything, or smashed anything. I don't get that angry, just frustrated. Then I stop playing.

The first thing that instictively comes to mind when I think of Gamer Rage is someone getting seriously way to PO'd in an online match. But nothing is truly as bad as a frustrating single-player experience.
Someone above said Mario Kart. I too have raged on that game harder than I have during any of my Halo matches. In fact the only time I do rage is in single-player games.
The AI cheats. They are the game. They'll cheat if they damn well please. And we hate them for it.

Those stupid boss fights where they don't even give you a hint as what you're meant to do or a checkpoint!
