Rayman 2: The Great Escape.
I loved this game as a kid, and I still do. But one thing I hated about it: those black caterpillars. Hairy, black, disgusting crawlers...and the noise they were making didn't help either. They seriously gave me nightmares.
Once, my game crashed just after I got through a whole cave full of those beasts, and I had to do it again. Pure horror D:

The Cradle in Thief 3, especially the first half of it, despite the fact (or because of it?) that nothing really happens there. That level is pure awesomeness and scary as hell.
Also the pure awesomeness of pretty much every Colossus in Shadow of the Colossus. Clinging to the back of that giant eagle as it flies over the lake, trying to shake you off with loops and swoops...
