Ridiculous Combos/Move Lists - Seriously, it's high time to start giving me the ability to perform the move with the press of a button. And please, no sliding the directional pad around this time.

Enemies that can endlessly attack you because you can't recover - I think every game that has a certain atmosphere should do that thing where you get hit and flash for two seconds of invincibility so you can move to the other side of the room.

The "most powerful item" - ...that you can only get at the highest level... and where nothing poses a challenge to you anymore.

The hardest boss is fought halfway through the game. Sick of this, make bosses challenging please.

For some reason, developers always seem to make their games so poorly that I lose interest after only three months of uninterrupted playing.

This must be remedied, dammit.

I hate escort missions and timed missions and of course bad steering in foreced vehicle sections. This is if we aren't talking about specific titles here.

The one thing however that drives me to the deepest level of insanity is being lost in a linear game with no objective, no clue where you are trying to go, what you are trying to do or how you would go about doing it. That makes me loose my ever loving cool.

I hate games that when you complete them, you have nothing to do apart from playing the same boring game over again (ie. not good replay value)

For me it's the boss fights that drag on & on. You kick his arse three times only to have to repeat it three more.

These aren't in-game annoyances, but rather annoyances associated with playing games, but the OP didn't specify so whatever. Eyestrain and stiff wrists are the biggest ones for me.

Kingdom Hearts fills that hole for me. I've played it a few times, now I can't stand it.

Demyx, NEVER. EVER. PUT TIMED BATTLES INSIDE A BOSS BATTLE. EVER!

Sephiroth, really? Seriously? Is he that cool? Kefka, pls.

And the final boss fight... Jesus Christ, you have to go through six battles just to finish the game?! No thanks.

EDIT: Oh, another boss fight that makes me go for fuck sake... The World. At the end of House of the Dead 4, every time he comes up my arm hurts. :(

When playing Time Crisis 3 at my local arcade I have difficulty breathing and my chest feels like it's going to explode. I have to relax and breathe deeply during every cutscene to avoid injuring myself. I don't know why I get like that.

Escort missions where the person you're supposed to be defending rushes in front of you. Sit your fucking arse down behind that wall and wait for me to kill the enemies, damnit!

That, and mute characters with zero personality. The game developers always find excuses like "The character is supossed to be you." Shut up, you're just too lazy to make a decent personality and motivation for the protagonist.
