More than once I've seen rooftop guards in Assassin's Creed games be so desperate to investigate my presence they jump off their roof, misjudge the obviously-huge gap and fall to their deaths.

Worst NPC?.....Persona 4 MC. Seriously, whenever you see someone on the Midnight Channel is REALLY obvious who it is (mainly because you talk to that person for an hour that day and they say, do, or wear something that is then featured on the channel) and your character has the same repsone of "You don't know who it is, so you got to bed"
I've got a few, but sadly no videos.
Fallout New Vegas: Every now and then, my companion would go sprinting into the distance as if to say "The spell is broken, I'm free, I'm free!" only to return a few in-game hours later as if nothing had happened. Wierd.
Dead Rising: "Oh, you're trying to save my worthless life? Good, while you do that, I'll crawl into this crowd of friendly-looking zombies, maybe stand in the wayt when you're fighting a boss."
Saint's row 1: When driving, your enemy knows not what the brakes are for, it seems every single time there was a car chase, my prey sped up like they were going for warp speed, only slowing down when they crashed into something... which means if they never crash, you may be in for frustrating mission, enemy gang cars never giving you a much peace to catch up to suicidal fuckwit de jour. (final rollerz mission, for example)
MineCraft: Not that I expect genius from farm animals, but I think FIRE is something they wouldn't casually stumble into. I don't know how many have perished in my burning netherrock fences and lava well, but I've watched the stupid fuckers jump into the fire/lava when it was a block above stumbling level. They aren't just blindly walking about, they actually hop up to their deaths. I get free cooked steaks just by visiting the bridge I lined with fire.

OT: at some point in oblivion's main quest you infiltrate a cult and you are expected to sacrifice some argonian guy. I wanted to save him but after trying 100 different things it became clear to me he WANTS to die.
If you take the book first, (you are there to steal a book) a trap falls on him and he dies.
If you cut his ropes, he attacks the cult and dies.
If you try to kill the cult first, he usually gets hit by area of effect spells and dies.
If you kill the cult first and he miraculously survives and THAN cut his ropes, he sprints directly into the next room, which is full of more cultists and dies.
At that point I just gave up. Maybe if you kill all the cultists than clear all the other rooms than come back and untie him and than take the book, he makes it out. But somehow I think he'd still find a way to die. By tripping and falling on a dead cultist' sword or something.
Sorry to spoil the fun, but that's supposed to happen (you can save him if you run ahead and trigger it a second before he steps on it, but that just bugs him out and he runs into a wall).
Also, the 2nd one, EVERYONE in the Shivering Isles is INSANE. The NPC is named "Runs-in-Circles" for a reason.

npcs and doorways, i could write a book about that. I have been trapped in buildings by my companions so many times in fallout 1 and 2. Also any shooter where you have friendly NPCs and they not only snatch the only good spot of cover, they also don't do shit from there. And stand in the doorway i was just trying to get into to escape the machine gun fire.
