VATS + the corpse explosion thing is pretty gorey. Dead rising also gives a good amount of blood.
Bloody Mess with a few extra-gore mods certainly puts Fallout 3 in the discussion.

Left 4 dead 2 is probably wins for me, but an honourable mention goes to cortex command for it's great particle and gore physics.

When i was in year 8, 12 years ago i was introduced to this on PS1 - for its time the Gore was up there, but just the pure wrongness was what made this one a cut above the rest - just the characters alone were twisted, let alone the moves and finshers....

I just can't say gears of war 2... the gore in that game was just too fake.
Shoot something and it blows into small chunks and blood sprays.
It really does't have that "Holy shit" effect.

Now god of war... when you can see the "Gibbly bits" or insides of some animal pour from its body.
That has the "Holy shit" effect.

God of War when it first came out. One because it was by far the most gory thing on the Ps2, and secondly it was our first experience of what Kratos likes to do with his spare time and no it's not cooking

I haven't played it but I would guess Manhunt 2 based on what I've heard about it.
Games are never as good as the originals. Especially since the second has been watered down.

Look for manhunt kills, one for example is stabbing somebody in the eyes with a shard of glass.

It wasn't very realistic, but Madworld could fill entire olympic swimming pools with blood.
In terms of something a bit more realistic, I don't know, I'd go for God of War III, Gears of War 3, or Mortal Kombat 2012.

The God of war games I've always stood out as quite gory for me.

Warning the following example is a spoiler for God of War 3, read at your own risk.

When Kratos Ripped off Helios head. I've seen it done before in films and games, but there was just something about the way Kratos did this, that amazed and shocked me.
Not to mention
Posidens eye-gouging and when he slammed the door on the horsemans head.
That last one still amazes me at how violent it is. I still love it though.

There is almost no part of the entire game that isn't covered in intestines and blood. Presented by Milton Bradley to boot.

Cortex Command? It's pretty bloody ridiculous (ha-ha) when it comes to decapitations.

It may not be the goriest, but it's certainly has ridiculous amounts of violence.
Would you recommend this game? Your post is the first time I have heard of it. After doing a bit of searching around it looks really fun!

OT: I remember as a kid how gory Duke Nukem 3D was and I felt a bit naughty playing it. Now a days I am so immune to gore that nothing really stands out as gory. I watched Sweeney Todd last night, that was pretty gory, not a game though obviously, just my most recent gore memory.

The PS2 Punisher game was pretty gory, and awesome as hell.

I actually haven't finished The Darkness 2 because I find it too unsettling, funny because I love Manhunt 1 & 2 (No neutering on PC)

Hell, I'll add some cute to this thread by showing how not neutered Manhunt 2 actually is:

Going for Darkness 2.

You can rip people in half
You can decapitate
You can "wishbone" a human (full gore content)
Kill people by burrowing into there stomach and ripping out there chest.

There are some really wrong detailed scenes I will not go into but this game is really fun to play.
