For Me it would have to be Uncharted 2 and 3, near the end the difficulty started to piss me off and it felt like the story was getting lazy. Also there was less of those OMFG moments the rest of the game had.
Bioshock with it's horrible escort mission.
The last few missions of Arkham Asylum were great but the final boss fight was a huge drop in quality.
Splinter Cell Conviction forced you out of stealth and into gunfights near the end

Let's see: Dragon Age Origins completely crapping out once the Landsmeet ended, Knights of the Old Republic 1 AND 2 ending with boring boss fights after a grindfest through boring looking cut and paste environments, Bioshock's aforementioned escort quest, Mass Effect 2's human reaper bullshit, BORDERLANDS.

Far Cry and Crysis were big culprits of this in my mind. Although they didn't wait until "near the end", and went to hell about halfway through.

Once the mutants (Far Cry) and aliens (Crysis) who could absorb ridiculous amounts of bullets came into the picture, it removed all concept of strategy and turned it into an endurance test.

The ending of Fallout 3 felt "meh". I guess I was hoping for something a little more amazing than Optimus Prime.

kotor 2 takes the cake i would say...

however, there are others, such as fable 2, that was the most outrageous bullshit of an ending to a game.

Deus Ex: Human Revolution, last stage a bunch of nutso easy to avoid/kill normal people and 3 options to pick from each of which was the exact same bullshit boring cut scene with a different voice over.

I love MGS2 to death but killing two dozen RAYs at the end, 3 at a time, was pretty lame.

EDIT:

OH... OH GOD I just remembered the travesty that was condemned 2's final 20 minutes. WHY DID THIS THREAD HAVE TO DREDGE UP THOSE MEMORIES!!???!!?!?!?!

I'd say DA:O, KotOR II, Lionheart (the game just deteriorates into mindless battle after mindless battle in the end), there are probably more, but I can't remember any.

Tales of games: The first two acts are pretty well paced and reach a cool climax then they put in a needless third act that seems to involve quite a bit of meandering around and padding to reach the 40-50 hour mark.

The final boss fight is an extend series of quick time events; hugely disappointing after the action of the rest of the game.

Well since someone's already mentioned Borderlands... I will have to go on about something else

[sub]Fuck you non-existant vault bullshit[/sub]

Ummmmm... Shit endings, shit endings. Aha! Condemned 2! Case closed! I never thought Yahtzee could be so right when he's on one of his more OTT rants. Retarded gameplay shift, terrible story, horror goes out the window.

Maybe Skyrim? I have the final boss literally right through this door... Never bothered to finish it. Probably says something about overall quality i guess. Or maybe it says more about my (still ongoing) obsession with UMVC3

Dragonage 2. A huge drop of quality near the end . And the quality wasn't that great to begin with . I'll also have to add every resident evil game . I love those games but at the end everythibg just goes bat shit bonkers . Also i don't know if this counts but every WoW expansion after burning crusade.

KOTOR 2 unless you have patched it. I had to ask questions on a forum to make some sense of it when I first finished it.

Towards the end of VTM: Bloodlines a lot of the multiple choice sections dried up and it was basically pure combat.
