While I know this game I am about to name isn't the hardest (I mean, I haven't even played some of the ones named in this thread), Comix Zone for the Sega Genesis is definitely a beastly difficult game. The game was a platformer with some beat-em-up style fighting mechanics thrown in. Sounds easy enough, right? Well, first and foremost, you have one life throughout the entire game. There is a slight chance that the final boss will arbitrarily resurrect you, but other than that, dying once ended the game. Secondly, there are no saves and no continues, so unless you are playing on an emulator or something, you have to beat this behemoth in one shot. Thirdly, it is extremely easy to lose health. Even punching walls or objects that cannot be bypassed consumes some of your health. This game, while probably not the hardest of them all, is most certainly a contender for the hardest Genesis game.
