I have to third the first and last book in the Tawny Man series.

The death of Nighteyes was a hammer blow as was the Fool's brutal death.

But, it was the reunion of Fitz and Burrich and Burrich's death that got me the most. I know that something drastic had to happen because of the situation with Molly. But, what a way to go and so soon after finding Fitz again.

The end of His Dark Materials trilogy.

It actually crushed me.
This, also when
Scoresby dies and when the sail off into the world of the Dead
Also Of Mice and Men.
I read that chapter 4 times before I believed it happened!

The Time Traveller's Wife.

When Henry dies. The way it is written just makes it so abrupt and shocking, even though you know it's coming.

Gets to me every time I read it.
