Hmm I can think of a few.

The first one is from Kingdom Hearts 2. The first revelation involving Roxas near the start.
You find out Roxas, the guy you've been playing, doesn't really exist. He's called a Nobody, a creature which doesn't really exist. Doesn't sound disturbing but the way it was told and the attachment I had developed with Roxas really made it unnerve me.

Then there was Doctor Steinman from Bioshock. That guy was so messed up that when you confront him the scene almost made my eyeballs fall out and my fingers stop working.

The most recent disturbing scene was from Fable 2. I had just gotten into a special demon door and inside I found a nice looking house. But my dog was growling so something was up. When you go upstairs you find some suits of armor surrounding a bed with this weird glow. Out back you also find a cave full of these suits of armor. They don't move or do anything but the silent atmosphere, the dog growling, the unmoving suits and the weird glow.... it just creeped me out and I was so happy when I got out of there.

Fear 2 didnt really disturb me just made me freak out cause of my lack of sleep/living off energy drinks for the last 2 days. Started seeing stuff just a overall bad experience.

I was at my friends house playing Silent Hill: Homecoming. The schism-based death caught me by suprise...a lot.

Also, when I was playing Silent Hill 2 for the first time, I was about 7 years old. The first time you see Red-Pyramid. Drenched in blood, behind those bars. Just staring...no violent behavior, just standing there...and staring. It honestly made my cry.

Not sure how wimpy this'll seem; but the Barnacles in Half-Life 2. I honestly freak out every time I'm caught by one.

When I was 4 i tried out a game called "Myulk". The whole concept of the game is that you take over an old milkfarm and your goal is to make it profitable. That surely doesn't sound disturbing but the game contains a little extra present in form of a green monster. Long story short, the music, atmosphere and that green blob scared that little 4 year old shitless. Now, 12 years later I still find the game VERY uncomfortable.

My mom was watching me play during that scene, and she totally lost her mind. Hilarious.
Try fucking playing that with your mum! I'm playing a wholesome zombie spatter fest with gratuitous swearing with my mother and towards the end they throw in a mother fucking Oedipus complex. Thanks a bunch Sega and the reverse birth ending was the fucking cherry on top. Definitely the most disturbed I have been as a result of a video game.

I think the only times ive been freaked out in a game is proberbly the Shambler in original Quake. Also Doom 3 has some very freaky moments and jumpy bits.

Bioshock in places can be a bit disturbing but its not too bad.

Painkiller's town level has rather disturbing sound effects, in fact most of the game has freaky sound effects

S.T.A.L.K.E.R scared me when i came across the first mutant thing

Erm, Fallout 3 in the subways can be a bit freaky but not really so much i've been disturbed by it

And me! they scare me loads. I hate the looks of them and when they suddenly drop through the ceiling and grab you. I forgot about those.

Everyone thinks i'm weird and i'm told they're not scary at all but they are :(

Dunwich Building in Fallout 3, at midnight. Ran around and thought I had cleared the room, and right then, when I slowly turned around, a Feral ends up at point blank, filling the screen.

I walked out of my room and calmed down by the TV for 10 minutes while my parents wondered why I had screamed.

Prototype's Web of Intruigue movies. Medical experiments, bood soaked halls, torture, operations... ugh. I skipped a lot of them.

The first time Nemesis appeared in RE3 and completely mullers Brad Vickers. Was only 8 or something when I played that first time and had nightmares for weeks.

This may sound lame but Harry Potter and the Sorceror's stone, final level. I think I was having a panic attack TRYING to kill Voldemort but he just kept POPPING out of no where and startling me.

I never did beat that level. :p I have to this day very raw nerves. I jump at anything.
