How do you manage Fallout 3 on Very Hard?

I just play normally. I try to get to Our Lady of Hope Hospital ASAP to secure the crap load of Stimpaks there, but otherwise, I play normally. The game really isn't that tough.

i probably did a bit i remeber my friend telling me how hard the fire ant quest was i pwn some fire ants he was like your probaly on easy and then we check and i was on hard/veryhard i'm not sure which it was

That's because you only played for a day... once you get a good weapon, the game is smooth sailing from there. At first I couldn't travel past Megaton without getting destroyed and once I did three or four small quests and got a sawed off shotgun I just breezed through the game only getting hung up at Little Lamplight because Dogmeat kept giving away my position.

I just didn't like how absolutely everyone and everything was out to kill you. Besides, massive open world RPGs aren't really my thing, I just thought I'd give it a try.

If you're looking for a challenge I found out a while ago that Fallout 3 is not the place to look. One time I was playing on Very Easy just for the hell of it and in the options menu on time I accidentally changed to Very Hard. I spent about four hours playing the game and didn't notice a difference until I went up against a group of Death Claws and noticed that they were taking one extra bullet to die than they usually did. I looked at the options and was like "Holy shit. I've been playing on Very Hard this whole time?" There are many things to be found in the Capital Wasteland, but difficulty is not one of them.

I don't. It's not that I find Normal to be hard, especially- I only really die when my eyes are glazing over from repetition and I stop looking at my Endurance=2 healthbar- but it just gets silly on harder difficulties. I don't see the need to shoot an unarmed civillian five times in the head, just so I can roleplay as "Dick who kills everyone in Megaton (Except Moira, obviously) personally.
EDIT: Also, you don't know fear until you play MMM, and find your first pack of Night Ghouls. You don't see them, exactly, at first. You just get this chill running down your spine- something is wrong. But... what is it?
You see something out of the corner of your eye. Weird. This side of the screen is just a little bit brighter than the other... as if there is something glowing somewhere RIGHT NEXT TO ME AH GOD

Yeah, I guess for me I just felt like it was that teeny bit too quick, which means I didn't feel quite as rewarded when I hit level 20/30. Plus I feel empty killing muties and not getting a 'Ka-Ching!'

It probably comes from playing too much Oblivion before it, which takes forever to level up anything (except conjuration for some reason) and requires extreme micromanagement of almost every skill in the game to even have a chance against high level baddies... now that I think of that, I'll take Fallout 3's simple XP Ka-chings any day over all that bullcrap.

I don't know, I found the game far more fun when the challenge was a lack of resources rather than tough enemies. It's quite enjoyable to see how many missions you can complete in a row without restocking, sleeping or returning to your house, for example.

Without mods is ... unusual and not advisable. Any Bethesda game is glitchy and prone to crashing. Mods can fix that. Anyway, OT, I play WITH mods on Very Hard and experience mods because it just doesn't feel right hitting level 30 so quickly.

I too recommend Wanderers' Edition. Needing to eat/drink, severe lack of stimpacks, faster combat, and a whole crapload of other changes make the game super freaking hard.
