Shao Kahn on Mortal Kombat. Had to put it on the easiest setting. Most the time, I play on the given setting, changing the setting as I see fit.
Normal here, too. Mostly anyways...
If it feels too much, then I change to Easy or whatever is the easier difficulty. Very rarely I have to change it to Hard. So yeah, usually I roll with Normal

Depends on the game. Some games I'll play on normal first, ususually grand strategy titles where I've yet to have learned the ins and outs or games where I'm not quite familiar with the mechanics.
But for something like a sequel or a game in a familiar genre I'll usually go hard. I like a challenge and enjoy having to reload and rethink my approach towards a situation every once and awhile. All too often however 'normal' difficulty might as well be easy since developers and publishers are afraid any sort of challenge might scare their customers away it seems.

I generally play on normal.
When I'm doing a play-through to give instructions to my dad, I'll do it on Easy because I don't have a lot of time.

i play on the hardest setting i can cause i feel that's the most 'authentic' version. i 
whenever i have to play the game completely through to unlock higher difficulty levels, cause allways feel required to do it, and i feel like i'm wasting a bunch of time on fake versions while doing so, cause i am.

Casual first for Bioware games becasue I like to get the lay of the land and pay more attention to the story, than how I will survive in the next room wave of enemies.

I have a hard time beating it on Medium, but I have far less problems with, and have beaten it on, the hardest setting.
I don't remember what it is called, something like "Extreme Ultra Hard".
I some times beat "Extreme Ultra Hard" faster than I do on "Easy".

My method is to go with whatever the game calls its 'normal' setting and then work from there. Usually, with fighting games, I end up going to the lowest difficulty, simply because I'm bad at every fighting game I've ever played. Most other things, I stick with 'normal'.

I usually play on the normal setting, because I, for one, play for the stories. And I'd rather get through the game and learn the story, get it embedded in my mind and know what's going on, than going through on the hardest difficulty, and being stuck on one spot for an hour, hearing the same dialogue over, and over, and over again, and forgetting what's going on from the pure frustration of it all. Then I'll go through again on the hardest setting knowing what I have to do
It's normally my second outing on big strategy games that gets plonked on normal/easy. On my first I always go for hard, because I have a panic and think "but if I start winning it'll somehow be illegitimate and I'll of wasted my time!
