Do you take mercy on killing NPCs such as women, children, or helpless animals?
I do, its something I do instinctively.
Even when I'm on a rampage such as GTA IV, I spare the elderly and females. In my mind I know its just a mash of pixels with some audio thrown in but I couldn't care less.

Most current open world ames give you an ilusion of an open world game.
You can't really go into an elementry school and start spraying the place with bullets and young blood.
I follow the character I am playing. Sometimes I'm mean, other times I'm nice, but mostly I want to get paid. Therefore the 'being a dick to people' path is better than hoping for them to repay you by painting you a painting. Instead of being sold to slavers for eight hundred botle caps.

That's what I like about Bethesda's titles. You CAN do whatever you want, from slaughtering everyone in chorrol because the bartender insulted you to...

It depends on whether or not I care about the game. Some games draw me in and I act 'honestly'. For instance, I won't harm innocents in Mass Effect.

On the other hand, if I don't feel immersed, I'll just do whatever is most practically beneficial, because I won't care about the characters. Fallout 3 did nothing for me, so I just killed and robbed for personal gain. I was annoyed when the game wouldn't let me kill my supposed best friend while escaping the vault. I wouldn't even have tried in a game that made me care.

Mostly I just don't care, but when a game connects with me enough and has relatable characters it can make me act differently. For example in GTA 4 I would mercilessly kill anyone if I was bored.

In Red Dead Redemption however I can never get very far on my evil save, yet I have completed it twice as a "good" John Marston who didn't kill anyone who didn't deserve it. Killing innocents just doesn't sit right with me when I play that game, and I think it's because I connected more with Marston and the other characters than I ever did with anyone in GTA 4. When a game world becomes believable and I can actually relate with the characters, then I just can't bring myself to be an evil jerk any more.

But there aren't any children to slaughter in their sleep!
Look at the kids camp in Fallout 3! They can shoot at you but you can't?
Oblivion didn't even adress kids!@
