Also, messing with them using 4th wall is terrifying. One idea I have is them walking through a door, the screen goes black, and then says the arbitrary message for that system saying "game could not be read"

Preferably as you thought you were about to do something necessary, preferably far from a recent save point or something.

One scary thought is opening a door to see a wall of flesh instead of a doorway. You try to shut the door, but it rips through and pulls you in. As your screen fills up with the red meat and blood of the flesh, you fall out of a ceiling in a new area, and a horde of enemies takes notice of you. Real this time. Then, RUN LIKE HELL.
It would mess with me as long as I'm "what the fuck was that?", then I'd get a grip and piss myself laughing.

Wall of flesh? Not really scary for me. And I'm not really a retreat-guy. I like to go down fighting ;)
Well what does scare people? I find that something unstoppable and at the same time, seeming like it should be avoidable, is decently scary. Same reason the grudge girl makes me have a heart murmur everytime I hear an odd noise in the bathroom.

Really what I call a combination of the creatures from The Ring and The Grudge. Two scary women, and a fugly ass boy.
I have to admit I haven't seen either. Never watched The Grudge and fell asleep during The Ring...

In the background, the console digs around for personal information related to the player in the configuration files and whatnot, or perhaps asks the player for information they don't connect with coming up later.

Insanity meter adequately filled, it starts using this information to prove that the horrors in the game are coming on their way to get you, in your house.

Stop yourself, if game developers get hold of what you just said then I will find where you live and urinate on your front door.

How scary would it be to playing a horror game and a small map appears top right of the screen, at first you don't think anything is that peculiar, then you see a red swarm on the map, and your street name, then the swarm stops where your house would be.

It would be worse if they chose that moment to use your webcam. Imagine a window popping up showing you playing, it stays in the corner of the screen, you think its weird at first but then you keep playing. About 5 minutes your eyes glance back at your screen and you notice something on the webcam, a silhouette or figure behind you.

I would have a stroke.

It probably wouldn't be that hard to hook up a machine to make threatening calls to your phone or email, either. Fun for the whole family!

I'll start weather-staining the front door.

I have a list:-

1) You turn into Baldur from Too Human
2) A tiger jumps out of the wall at you, and as soon as it hits you it turns into a rabbit and runs away.
3) Enemies on levels that are set centuries ago turn into modern day soldiers.
4) You constantly morph into different animals every 5 seconds
