Hellgate: London, because of the funny quests (Chocolate Park, hehe) and the good gameplay.
Really? Good gameplay? I think you're one of the first to say this.
I actually think it plays much better than Diablo 1 or 2.
Im slowly creeping up on the level cap with a grenade focused marksman and now that Flagship has fixed most of the bugs (at least the really big ones) the game is pretty fun. Especially if you play with a friend and pick complimentary classes/abilities.

I go through my favorites every now and again: Starcontrol 2, System Shock 2, Super Metroid, Doom & Duke3D ect. Really good games remain interesting and fun nearly infinitely as long as you give them a rest period.

Zelda: Link's Awakening, it's not the game I've beaten the most (Star Fox 64 and Yoshi's story are currently having a jousting match over that matter) but it's one of the most enjoyable games from my youth. Also, replaying it was just another excuse to use the flying rooster boomerang combo of death again.
Eurgh, Yoshi's Story. I played through that game so many times, just because I could do it quickly.
Anyways, I'm easily amused, so I'll replay most of my games. The main requirement is having more than one save file. I don't like deleting my first play through of a game at all.

i've played through eternal darkness on the gamecube with all three starting artifacts, just to see the extra ending.

plus its an awesome game, and eternal mode is fun.

Actually, in Mass Effect,it really encourages a replay, 'cos of the achievements. You can't get 'Extreme Power Gamer' unless you play through with a character twice, and could you possible play Insanity with a fresh character? From what I've heard, i6t's a nightmare. That, and it's fun playing through the different classes.

I've noticed that I have to wait a few months before I can tolerate playing through a game once again. Usually I play games in cycles were when I get bored of the game I'm currently playing I look through my meager library and pick a game to play over again from the start. All in all, I must have played through every game I own for the new consoles once, and for the old generation each got an average of 2.5 playthroughs. Although some games were too annoying for me to stand playing through again, so I just kept going on my main save file. I'm looking at you Morrowind , you know what you did wrong.

Insanity mode goes beyond nightmare levels. Far, far beyond.

I replay my copies of FF7, FF8 and now FF9 a good few times, and it's a nice little test to see if I can remember how to do everything, how to get the weapons, and if it's just a fluke I beat some of the side bosses. Replaying Okami was also a challenge of mine, to see if I could get through one of the digging games without throwing my controller at my TV.
