Right, so if in ME 2 you got the ending where the entire team, along with Shepard dies while destroying the collector base? Who will you play as in ME3. I never finished the ending with Shepard dying, all 3 times he survived with the whole team, bar my first attempt in which a few died, but all 3 Shepard him/herself survived

That and make all the wrong choices during the last mission. Ordinarily I say there's no wrong way to play a game, but if you're rushing through that quickly I have to say you're doing it wrong.

So like putting Grunt in charge of the Tech mission, and having Jack lead the other squad (hehe, who would follow Jack?), stuff like that?

It's extremely difficuly to get the "Shepard dies" ending because you have to end up with less than 2 of your squadmates alive. For that to happen you basically have to do no loyalty missions, not upgrade your ship, and make all the wrong choices during the suicide mission. You have to really, really suck at the game.
