Regulations make this sort of thing more likely. Each person along the production line just assumes the regulation caught the problem in the previous step so no one actually pays attention. Such is the unintended consequence in thinking a government can actually accomplish anything people should do for themselves.

Could have been worse, it could have been inside the loaf and he could have taken a nice big bite of it before noticing what the crunchy bits were.

Also true, and quite disgusting. I can't not imagine the crunch now.
