Medical technology in Europe was held back a thousand years because the church considered the work of Galen and Hippocrates to be irrefutable. It is only since secularism has taken hold that scientific and medical breakthroughs have been discovered.

To be boring i'd say it's our worlds dependence on oil. But then again i think that anti-biotic resistant strain of an airborne bacteria could do some serious damage.

I'm not tooooooooooo worried about aliens because humans have been trying to kill each other since the beginning and we've gotten pretty good at it. If aliens came to earth they may not have that nature and could have focused there knowledge on other things.
