Yeah, you just need to get to the final stage after getting the IFF asap. I think all I did was gain Legion's loyalty and then I headed through the Omega 4 relay and was able to save them. I was able to finish the last level without anyone getting killed. It's very important to full upgrade your ship and give people their personal upgrades like Mordin's Omni tool. You also have to send people to the things they'd be suited for based on their leadership and abilities. You want the best biotic for bubble so tack Jack, you want the best leader for the distraction team so take Garrus, Miranda, Jacob or maybe Zaeed (not sure on him) and with the tubes I found Legion to be the best, Tali got shot but I also sent Samara (a poor leader) as the distraction team so that might have been why. I don't think that it matters who take with you to the final fight as long as they are loyal.
I almost thought so. The only reason I didn't play ME1 is because I didn't have the right computer at the time. That was my ending as well. Only it took one more try. (I didn't realize Jacob wasn't competent enough a "leader" to keep Tali from eating bullets. Which is unacceptable; Tali is awesome. I also didn't know that I had to put off doing Legion's loyalty quest and go save my crew right away to keep half of them from being liquefied. Especially Kelly. I had to save her dammit!)
But seriously, it's called a suicide mission all through the game! How can people not prepare their crew's loyalty and their ship's armor, shields and cannons? It's obvious that that stuff is going to determine how many people survive!

And I read on the wiki that for every major upgrade you don't do for the ship, a squad member bites the dust from a hull breach, debris collision or some such.
