After replaying Mass Effect 3 the only thing I can think of is that the company and writers that made the rest of the game, and paid so much attention to detail, is at least smart enough not to make mistakes that big in the last 5 minutes.

Well it's pretty easy to screw up the last 5 minutes of your game when your game's director decides to go on a cocaine-fueled bender and absolutely butcher the ending of the game by chopping out 80% of the pertinent information that your writers wanted to include all because a legless stripper refused to give him a lapdance because he wasn't wearing any pants at the time.
It is, but it's much harder for everyone in the team to not call them out on their bullshit before they go through beta testing and shipping.
The ending is too full of holes for nobody on Bioware to not say anything. Something's up, I can feel it.
Don't get your hopes up. There's a reason most people stay silent, and it's way simpler. They don't want to risk their jobs. It's pretty much the exact same thing in the movie industry - there are so few cases of people calling movies they've worked on crap (or otherwise tarnishing it or people related), that you could probably count the amount of them on your fingers.
Basically, if you talk shit about someone, they're not going to want to work with you. And along with the sad reality that this is the status quo in most development studios, you either stay shut, or greatly diminish your chances of having a successful career in gaming.
Edit: Oh, and that thing is so easy to fake that you might as well consider it that way on its face.
