During my second playthrough of Bioshock Infinite Elizaeth's skirt went completely nuts and started clipping through her legs every time she stopped moving. This made some of the story sequences a bit hard to take seriously.

One time a friend of mine was playing Skyrim on my computer and was fucking around in Whiterun with lightning magic (so he could be like Palpatine). He heard an elderly woman yell something alogn the lines of "No! Stop!", and he promptly turns around and kills her Episode VI style. Then she began to start levitating in the air, and caused the entire game to force crash.
Skyrim- I had a dragon not give me a soul(nothing new)
The body of said dragon decided it was a good idea to follow me around, every time I fast traveled or exited a building/cave to an open world he was there.
It got rather annoying in Morthal when you exit a building there is dragon caucus rotting in the entire staircase.

i once went into ragefire in WoW (it's ragefire right? the instance in ogrimmar? germans got translated names ect and it sucks)

Playing a mod that upgrades the Volkihar Castle player home after finishing Dawnguard.
Go to the Volkihar Docks to upgrade them and talk to some new quest character.
There is a combination of about 30 sabre cats of various kinds, bears of various kinds, and wolves of various kinds attacking the quest character at the docks. They are all piled into a fairly small space so it just looks like a writhing mas of fur and claws.
I start laughing out loud at the ridiculousness of the situation.

IN VTM:B the elevator buttons got replaced with a beer bottles for some reason. Fortunately it fix itself when I reloaded.

When I was playing Assassin's Creed III, I saw a horse disobeying gravity once, just floating there, and I calmly walked away. Called it a animus glitch and got on my merry way.

I remember glitching a Daxter section in Jak II...
Also, more recently, I've been playing Proud Mode in Kingdom Hearts 1.5... and if I kill a heartless near a certain "wall", any munny, HP balls, or MP balls (especially munny, since most MP balls do come back out of the "wall") get stuck on said "wall"... (That NEVER happened in the original PS2 game...)
So, when completing Mar's challenges in the tomb, there a part where Jak and Daxter get sepparated and, when you play as Daxter, he's running away from a giant spider... Usually, if this spider touches you, you die and reset back to the last checkpoint... However, while I was doing an LP of this game on Hero Mode, the giant spider missed hitting Daxter and crawled right passed him before freezing in place due to how the spider was programmed to do whenever it did surpass Daxter's limb body, I assuming... So, I end up continuing the section as a more less urgent place with a paralysed spider behind me...

OT: I had one in New Vegas (Didn't everyone lol) that involved a dead grandma falling from the sky after I exited a building. It was pretty hilarious.
While I never (directly) saw it myself, I was once playing Pyro on TF2 (MVM) and my teammates said my head had vanished entirely. I can only imagine what they were thinking as they saw a headless Pyro charging around immolating entire hordes of robots.

There was the one time I met Scuttlegoat. I was just minding my own business on the slopes of the mountains outside Riverwood, heading for Bleak Falls Barrow to recover the golden dragon claw, and who jitters across my path? None other than Scuttlegoat!
He seemed to be completely at ease,
but his legs moved as fast as I could see!
Scuttling across the mountain path,
he made me cry, he made me laugh!
His furious legs moved in a whirl,
still he moseyed by without a care in the world.
His ridiculous gait was made more bizzare
by the fact that he seemed unaware so far.

And then I remembered I came here to Smash!
And so naturally, I had to kill poor old Scuttlegoat.
His meat tasted like any regular goat to me.
He seemed to be completely at ease,
but his legs moved as fast as I could see!

Unsure how it happened, but one time during Mario All-Stars, i played Mario 3 and had what i called a "Koopa Boot", it essentially worked like the Kuribo's shoe, but the tileset on the boot itself seemed pretty warped, including the palette. maybe the cartridge was dusty, or just old...still weird to use a boot in a castle level.
