I have to agree with the knife-dildo from Se7en.
Also, various death scenes in many movies, such as A Clockwork Orange and Saving Private Ryan, were pretty hard to watch.

The deaths in the Hostel movies. Namely the Japanese girl who got her eye burned out and the sadistic wackjob who got his genitals removed.
Nasty.

Also gotta mention the guy in Devil's Rejects who was knocked to the ground by Otis and had his face cut off while he was still alive, after which Otis wears it as a mask to terrify that man's wife.
Man, I really hated the Firefly family.

Edit: Actually I think "Bad Taste" might top that with the guy chainsawing himself through that alien's body and coming out the bottom... Sir Pete Jackson's splatter work in all it's glory.

I can't believe it hasn't been mentioned yet

Higurashi no naku koro ni

Filled with bat shit crazy characters violent deaths and they don't just die once they come back and do it again. Also makes extremely good use of both emotional, mental and physical torture in various degrees.

Shion having to rip off her own fingernails or have her friends be killed


Rika tries to inject shion with a syringe ends up getting injected herself and decides to kill herself by stabing herself in the neck repeatedly

In The Green Mile, when Del gets electrocuted, it's horrifying by itself, but if you watch the whole movie, and see how he befriends and trains the mouse, it makes you weep.

Anyone mentioned Human Centipede yet? Girl has her face sewn to a guys ass, and he dies while She's stuck to him. To make matters worse the girl sewn to her at the back also dies, and the implication at the end is that no-one knows she's there.

There was this made-for-TV movie I saw on the Disney channel once, maybe ten or so years ago, about a mummy who wakes up in modern day New York or something, and he befriends this group of kids. Early in the movie, a few of the kids are watching a slasher movie in the theater, and in that movie this guy is cleaning dishes in his kitchen when the villain busts down the door and attacks him. The guy drops a knife into the sink, and it gets caught in the garbage disposal blade-up, spinning around like a drill. The slasher grabs the guy's hair and starts forcing his face down into the sink. Then it cuts to a close up of the kids, the movie-guy screams, and the theater is bathed in red light from the screen.

Pretty violent for Disney. It still makes me cringe thinking about it.

That and the subway scene. Also not a death of a person but Caleb seeing all the wildlife die in his dream/vision.

Talking of wildlife dying, Bamby's mum dying used to disturb me a lot when I was a kid, probably would now to if I watched it.

I haven't seen that movie, but dying forgotten and alone is a fucking horrible way to go.

OP: In Farscape, an alien is killed when he falls into a vat of acid.

Also, though it isn't technically a death, I remember part a movie I watched when I was a small kid. In it, a girl falls headfirst down an abandoned and collapsed well and is stuck headfirst underneath its crumbling stones, upside down and unable to call for help, and dozens of metres below ground.
That scared the living shit out of me back then.

Hey, that 1st not is not a death.

And higurashi had much more disturbing deaths.
